Prolia vs Zometa

For 3 years I was on Eligard/Xtandi and was receiving Prolia to help keep bone health. All good, undetectable. Then an 18 month vacation, which I am coming off of. All treatment to be the same as before, except th MO wants to change from Prolia to Zometa. Anyone use both of those, Prolia and Zometa? Experiences?

Hello,

I would want to know why he (or she) is recommending you switch from Prolia (denosumab) to Zometa (zoledronic acid). I have been on Prolia for over ten years with no side effects. Prolia also requires less monitoring of kidney function than Zometa.

I think it is because there are two small spots to be radiated. One on a rib and one on a vertebrae. But you point out my concern - prolia was fine and I had no issues. This other drug has more potential for issues so I am hesitating a bit

I have been on xtandi for 5 months and so far no side effects. I do a lot of exercise with heavy weights & swimming. Eat only good healthy food and have a great wife so maybe all this is why I have no bad side effects

Free advice.... don't go swimming in the ocean wearing heavy leg weights.....If you decide to... make sure your wonderful wife is the beneficiary of your Life Insurance. Yiasou!!!

After 4 years on denosumab I was switched out to Zometa. My MO said that there was a max time recommended for deno, and that Zometa was required to help stop the snap back effect of ending deno.

I handled the denosumab pretty well. No real issues. The Zometa gave me a 103 fever the next day and some bad gastrointestinal issues for a few weeks.

You can also accomplish that with an inexpensive oral regimen of weekly alendronate pills for a time.

You already know you tolerate denosumab well. I would decline to switch to Zomets. However you could intensify treatment from the Prolia dosing to the Xgeva dosing with a 3 month interval rather than 6 month. It is a fine drug to protect bones as long as you keep your dental care up to date.

Thanks I did call the MO and tell him, based on what I am hearing, that I'd prefer to stay with densumab. The more I read the more I see experiences like V10fanatic.

Additionally, there is some suggestive evidence (not a clinical trial but secondary evaluation ) that denosumab may alter the bone micro environment to make it less susceptible to establishing metastasis to bones. That is compelling for me as I have no bone Mets and want to keep it that way. MB
