Xtandi has failed we think

Hi, back again

Ok so the psa has come in and its jumped from 12.8 to 34 in about 7 weeks. Testosterone 0.03

January 2023 with psa of 83 ( this was on a watch and wait process l was started on Zoladex again but after reaching 7.0 psa started rising again, Testosterone 0.02 after 6 months.

l was diagnosed as non met castrate resistant, by now l was referred to an Oncologist however the Cancer Agency lost my records on there computer system during renovations Big delays ocurred.

Finally l saw my Onc in Nov 2023 and was told that l would stay on Adt for life but he would add Xtandi to the mix.l decided that l d idn't want to be on adt injections for life so l made the decision and had a Bilateral Orchiectomy in March 2023 that went well.

November 2023 put on Xtandi 160mg, have suffered dreadful side effects. Blood Pressure skyrocketed Xmas Day now on 2 BP meds, Onc dropped to 3 pills, then to 2, then back upto 3 horrendous headaches. Stopped taking meds on Monday this week cannot function with the headaches.

l had bloodwork done Monday as well. Psa as stated above has gone up, l am having a conversation with my Onc on Friday this week on what happens next. Is Daralutimide a good choice next?

Been doing this since 2013 had some good years and bad but l keep positive and try very hard to exercise on my bike as much as possible.

Any help, advice on next step would be useful.

Nubeqa is somewhat similar to Xtandi. If xtandi has ceased to work the next possible steps are docetaxel with a steroid or cabazitaxel. The other options are provenge or keytruda (immunotherapies) or certain target therapies if there is a cancer gene profile that indicates they will work, and then there are radiotherapy options like pluvicto.. Another option is getting in on a trial of ARX517 as at least one member here seems to have failed Xtandi and responded on this trial.

Thank you for replying l am in Canada and there are no Trials for ARX517 as far as l can see. Maybe in the near future. In the meantime l think l will try Nubeqa and keep fingers crossed it lasts longer than Xtandi but with less side effects. Keep the faith and Cheers.
