My husband's cancer is being restaged. PET scan: Definitely castrate resistant. Diagnosed liver biopsy today confirmed "poorly differentiated neuroendocrine carcinoma." Tomorrow he will have an ultrasound on a tumor in his kidney and FGP PET scan, and radiation consult.

He has been experiencing significant bone pain 10/10 - first in his neck, then his left side under breast, then left shoulder, last night right lower back. I'm talking cry out loud pain. Getting little sleep. Two grueling 12-13 hour visits to ER.... pain being treated with Ocycodone, Muscle relaxer, Tylenol, Lidocaine patches. It seems to me like the pain just keeps moving. Get it under control in one area and it pops out in another. This has progressed rather quickly in past two weeks.

Oncologist wants to start chemo - Cabazitaxel on Friday. Is planning to do the chemo in the hospital where he can be monitored afterwards. We still have tests to do - as detailed above. But he is willing to get going as "something has to change."

Are we moving too fast? Is this the standard of care at this point? (He didn't have chemo at diagnosis as he was just too sick. Recovering from his CKD - stage 3B. He also has Bradycardia, high blood pressure.

I had been keep careful notes from what I learned on this forum. My computer crashed, Lost everything. I do recall reading about this small cell neuroendocrine and hoping that I never had to learn more about it.

I haven't had time to do the research and it is making me very anxious. At the same time, I feel the urgency------the crys out loud because of the pain - make me want to cry out loud.

Advice is needed. You've been very helpful in the past. Thanks in advance for any input.

Discuss adding carboplatin to the cabazitaxel.

The Lu 177 Basket Trial hasn't begun yet. Hopefully he will be able to join it when Chemo concludes.

He said he will be getting carboplatin and etoposide because this appears to be transformed disease to small cell (neuroendocrine). If this was aggressive variant prostate cancer without transformation to small cell, the I would have recommended carboplatin/cabazitaxel so carboplatin would have been included in either regimen chosen.

I have stage 3 high grade serious incurable cancer that's spread. Had my chemo in 2019 long sessions on Taxol & Carbo l must admit it was tough at times l also have a managed brain tumour but the only real time l felt it was 3/4 days after felt worn out but gradually that wore off thank goodness l rested when l was tired but always ate well to keep.my strength up and nearly 5 years later lm still Ned so thank god doing well. It did take it out of me but l was determined to get through itSo please god you will be the same. I always stay as positive as possible and believe that l can beat this. Having nursed my husband through pancreatic cancer. That was a really hard time but over 30 years later he's helping me in my battle Stay positive keep well dont google and get 2nd opinions if necessary. Love Sheilaf xxxx
