Testosterone is increasing? Should I be worried?

Hello everyone,

Since my father’s surgery in 2022, his PSA levels have been undetectable. He receives a Zoladex injection every three months. Until this month, the reference range for total testosterone was 1.93 - 7.40 ug/L, and his result was 0.126 ug/L.

This month, the lab's reference value format changed. The testosterone reference range is now 150-684 ng/mL, and his result was 10.17 ng/mL.

What is the difference, and should we be concerned?

Testosterone feeds cancer, Testosterone number looks good. many warriors in this group has more than 1 oncologist. Please seek for second opinion as well.

I strongly disagree with dr based on your BIO information.

He should be taking enzalutamide, apalutamide, or darolutamide with ADT. He already had chemo, so he should have more success with ADT+ enzalutamide, or apalutamide or darolutamide

It is also a good time to ask for a PSMA PET Scan as well.

He should Also take enzalutamide, apalutamide, or darolutamide with ADT, That's call triplet therapy and also have better result.

Does he has any Germline or Genomic Mutations?

He has an MSH2 mutation.

may be PARP inhibitors also help him in future.

Move the decimal point over 2 values on the ug/L, that will equate to the ng/mL measurement,

10 ng/mL is castrate level,

3 decimal points. 1 ug = 1000 ng

I think you got the units wrong. His result was more probably 10.17 ng/dL. The previous result of 0.126 μg/L = 12.6 ng/dL, so both are about the same. Anything below 20 ng/dL is excellent.

Learning questions - Here goodgoal posted his/her dad only taking Zoladex ever three months and He also completed his docetaxel chemo but did not prescribe him AR at all. do I miss anything ? It is not standard treatment also he has MSH2 mutations as well. What's best thing to do ? i

In 2021, we didn't have the info on triplet therapy yet. goodgoal had the SOC for newly diagnosed metastatic PCa then which was either ADT+chemo or ADT+Zytiga/Xtandi/Erleada. It is not something that can be changed midstream because chemo (and ARSis) create senescence. While PSA is so low, there is no benefit to adding an ARSi now.

Remembering the GOOD OLD DAYS (?) back in April 2015 on a Monday urologist tells me SOC is ADT and blah-blah-blah but I said "out with the boys" so 24 hours later they're GONE. 😁
