Jevtana and carboplatin

I have done 10 rounds of Docetaxol, had a PSMA scan which showed progression in a lymph node ( I only have lymph node disease no bone or organ mets.) I started Jevtana and carboplatin about 9 weeks ago. I have what is described as" PSA surge". When I started my psa was 4.4 now 8.3. Has anyone else experienced this? What if any thing was done?. My Mo says stay the course, scans to be repeated in July. Thanks for all your help

I am NOT doctor. my advices are base on search and learning from other comments.

I am not sure whether keeping Jevtana and carboplatin for future is good idea or not.

Also same time I think it is good idea to consult radiations oncologist. Many times there some spot very hard to work so radiate them is good options as well.

In my treatment plan, I have oncologist, radiation oncologist and primary care. my goal is to work with everyone and understand treatment plan, side effects and make best decision.

Do you have options of lymph node surgery ? Search in group. There are many example of lymph node surgery as well.

Thank you for you helpful response. I have seen a RO at Fox Chase. Cannot radiate because outside of safe zone to close to heart.

Do you have any Germline Mutations or Genomic Mutations?

If it is a PSA surge from dying prostate cancer cells, it should go down in a few weeks, Or maybe some of the metastases are responsive to chemo and some aren't. Your scan in July will let you know.

You seem like a good candidate for Provenge and Pluvicto.

Thank you sir. I did Provenge last January and started chemo in September. I tried to get into a Pluvicto trial pre chemo but since I failed Xtandi I was rejected. I am sure I will qualify if the scans show progression.
