After Lupron, NUBEQA and Zytega … and a recent FDG Cat scan Several nodes lit up in the periodonal aorta area of his abdomen.

Being totally right up against the aorta, a decision was made to remove them robotically. Surgery went well. All lymph’s were removed that lit up on the scan.

docs felt PSA would reduce drastically since lymph’s were only thing that showed up on scan.

However, 3 weeks after lymph node s removal PSA has increased… it was 25 before identified lymph were removed and now PSA IS slightly up to 27!

He has CDK 12 mutation and AR-?

Would chemo be best next option?

I’ve read mixed reviews of Keytruda and another immuneo drug… may have potential.

I’ve also heard BAT has worked for some with CDK12 mutations!

Any experiences similar to this?

You can try BAT, after two cycles you will know the outcome.

During surgery it is difficult to detect which lymph nodes are affected. So probably not all affected nodes were removed. I suggest a PSMA Scan which is more sensitive than an FDG Scan. It will show the reason for the PSA rise. You wrote an FDG scan was made.

He had several PSMA SCANS and they showed nothing. Finally the scan with contrast is when areas lit up!

Then he has PSMA negative tumor. The FDG PET will not detect all of the mets.

My dad never opted for Chemo. BUT lived for 17 years with prostate cancer, and 3 years with it very advanced and lived an optimistic, and quality life.

He always told people to "always have a plan of something to look forward to" and he really lived that way.
