Lynparza has almost no effect on ATM and would be very toxic to add to chemo.

Did you get the results of your liver biopsy yet?

1st line of treatment for NEPC/SCPC is platinum based chemo, which you are starting now.

When that fails, 2nd line is Clinical Trial or Taxane based chemo (Docetaxel). It is better to pursue CT earlier rather than later.

Most CT trials today for NEPC/SCPC are testing combinations rather than monotherapies, so look for a treatment that pairs immunotherapy (ie Keytruda) with another drug.

In addition to keeping you alive longer another possible outcome is to re-sensitize the cancer to respond to chemotherapy again.

The keyruda showed it failed in trials,

I think he is giving me cisplatin and Docetaxel Saturday

I’ll find out

What other drug should I be on?

I have no symptoms and the biopsy is not back

I can't advise on what treatment to follow. Talk to your MO or get a new MO if you don't like/trust your current MO.

I can point out that Platinum chemo is standard 1st line for NEPC/SCPC and that cisplatin+docetaxel is a SOC option. So you're in the ballpark for your next treatment.

I am pursuing a phase 2 clinical trial, pembrolizumab+lenvatinib, as 2nd line treatment for NEPC/SCPC. I have just recently failed carboplatin+etoposide (1st line) after 3 infusions.
