Meta analysis of Eastern Cooperative Oncology Group(ECOG) PS predicts OS regardless of category, previous chemotherapy, and mPC population

”Overall, 75 studies, comprising 32,298 patients, were included. Most studies (72/75) included patients with mCRPC. Higher ECOG PS was associated with a significant increase in mortality risk, with the highest estimate observed among patients with mCRPC with an ECOG PS of ≥2 versus <2 (hazard ratio [HR]: 2.10, 95% confidence interval [CI]: 1.87–2.37). When stratifying by study type, there was a higher risk estimate of mortality among patients with mCRPC with an ECOG PS of ≥1 versus <1 in real-world data studies (HR: 1.98, 95% CI: 1.72–2.26) compared with clinical trials (HR: 1.32, 95% CI: 1.13–1.54; p < 0.001). There were no significant differences in the HR of OS stratified by previous chemotherapy.”

Note that Grade 1 of the ECOG performance status is:

Restricted in physically strenuous activity but ambulatory and able to carry out work of a light or sedentary nature, e.g., light house work, office work

Note that Grade 2 of the ECOG performance status is:

Ambulatory and capable of all selfcare but unable to carry out any work activities; up and about more than 50% of waking hours

Honestly, I only understood the last sentence.

Basically the study shows an association between mCRPC patient survival and ECOG performance status, I.e. the more disabled you are the greater risk of death. Whether you had chemotherapy or not didn’t appear to change the odds.

One might question whether how far into the ECOG 'negative numbers' one was when diagnosed may be worthy of another analysis. "Coming to Jesus" when diagnosed may be less a factor than what one may have 'put into the bank' over a lifetime. I'm convinced that where I started as a runner/biker/active leaves me with a lot more to work with.
