Hi all, a CT scan 5 weeks ago showed a spot 1.3 cm. A second scan last Friday showed spot progress to 2.1 cm. Curious as to thoughts and suggestions.

Can it be biopsied? Have you done Jevtana+carboplatin?

Have not done Jevtana + carboplatin. Next appt Aug 17. My onc sending results to OHSU where I just finished the vision trial.

Biopsy could be important to see if the cancer has mutations which could be treated with specific medicines such as Olaparib, Rucaparib and or Keytruda. Besides systemic treatment with ADT and chemo, liver metastases could be treated directly with SIR microspheres.

Hey Tango, is chemotherapy required along with SIR-Spheres for liver mets? Or can SIR-Spheres be done by itself for liver mets?

I do not have clinical experience to tell you one way or the other.

My guess is that liver metastases are usually present in patients with multiple bone and/or lymph node metastases. Some may have neuroendocrine PC. it is a systemic disease and an effective systemic treatment should be advisable.

Can I ask what your current psa is?

My current PSA is great. It is .081. When I started the vision trial last year it was 375. These new spots on the liver do not seem to have any connection to the PSA, and that’s what scares me.

Your MO will most probably schedule a biopsy for you. I went through it twice. I have and had lesions on my liver as well as my lungs. I just started another clinical trial. The first one of olaparib and cedirinib combo stopped working after two years. The liver lesion and lung nodules started reappearing again. So hence this new trial. I refuse to do chemo again. Just won't do again. Even if it's my last option. Good luck in your decisions.

I’m probably one of those who kind of just goes through life without too much thought about specifics. For the Keytruda trial I don’t remember what I needed and for the vision I had to have a failed chemo as well as a failed enzalutimide. My cancer also had to express PSMA.

I don't know if you did go through the gene screening process. I did about year after being diagnosed. It was not determined if I was carrying any of the Braca genes but I do have a family history of cancer. I qualified for both trials without regarding any genetic mutations.

What is your Gleason? Did tour PC biopsy mention Neuroendocrine? My husband is Gleason 10 with Neuroendocrine PC. He did well on hormone therapy for 15 months. Then it started to spread. It went to several bones and liver. We tried a clinical trial....it did not work. Now he is doing chemo....Carboplatin and Etoposide. We are hoping to kick it back and find out next Step. Prayers and positive vibes sent your way. 🙏👍

I thought I replied to this but apparently not. My apologies. I was diagnosed Gleason 9 fifteen and a half years ago. I’ve had 40+ rounds of radiation, 13 rounds of chemo, Keytruda clinical trial as well as vision clinical trial. PSA as much as 375 (at start of vision trial) and currently at .81. Thanks for the prayers. They are the strongest tools in anyone’s arsenal.

Wow! You have been through/done so much! True inspiration for us all! May I ask where you are being treated? By your local oncologist or a large cancer center? We will be needing to travel as our local oncologist....this is waaaaay over his head. So, I’m trying to research and find the best option.

My local oncologist is in Medford Oregon. I like him because he is not afraid to ask specialists. He and I are both consulting with Dr Julie Graf at Oregon health sciences university. She is prostate specific and her department head is Thomaz Beers who is very well regarded.

Jim , I’d just like to say that you both look handsome .You in the suit .. I m sorry for this liver involvement . That sucks. I’m pulling for you to over come it .. Scott 🙏

No biopsy done yet. I have an appt with local oncologist on Monday and OHSU on Wednesday. I’ll post after wednesday what I learn.
