hello. I Had surgery June 13 2023 biopsy showed Gleason 8. Psa 13 perineal invasion seminal vesicle invasion post surgery pathology Gleason 7. First two ultra sensitive psa came back with less than symbol 0.02. Then my last ultra sensitive psa came back 0.02 without the less than symbol. Is this something I need to be concerned about. I am just seeing the urologist that did surgery. Should I get a med. Oncologist onboard. Age 71 thanks in advance

No need to do anything unless PSA reaches 0.1 or 0.2 if it ever does.

Mine was the same . After RP was 0.014 then 0.010 then 0.013 then 0.020 then 0.023 then 0.020 26 months post RP. Hopefully stays where it is . Nice if was 0 but as long as stays where it is. Listen to Tall Allen he has been amazing support and source of information to keep all in perspective.

The first reliable landmark is 0.06 which is considered the pivotal point ( i.e. 50-50% probability) of an impending BCR. Until then, just monitor.

Hi Justfor_ you have similar pathology to my husband. He went from 0.01, 0.02, 0.03, 0.02, 0.04 now 24 months post RP. Do you have a link to refer to the 0.06 point? Thanks

A 0.05 followed by a 0.06 is the earliest pivot for a 5-year BCRFS from <50% to 50-75%

Thanks. I'm confused where it says prob is >90% for progression free if PSA is >0.4? Isn't PSA 0.4 not good?

It is entirely possible that the "<" symbol was somehow left off the transmitted result, whether entered by a human being or via an automated system.

thanks to all who replied. When I went in for office visit with urologist he started with worse case scenario and said nothing about that it is still undetectable. Needless to say I wasn’t in a good place after leaving. Thanks again everyone not sure what I would do without your help and support

I am a patient at MD Anderson and they won't test at less than 0.1. IOW , everything below 0.1 is noise. Right or wrong its part of their SOC. I wouldn't want to fret over such small changes anyway. After the prostatectomy mine went straight to 0.3 within a few months but I knew that was coming based on the post operative pathology report. Then did SRT / 6 mos Lupron. Last radiation treatment was 6/19/23. testing every 12 weeks and still <0.1.
