Ureteral cytosis

Started Jevtana in August hoping for more moderate side effects and efficacy than I'd had with three prior rounds of Docetaxel. First Jevtana round was without carboplatin. Second Jevtana round had carboplatin included. Hematuria got a bit more severe and then two weeks into the three-week cycle, I was hit with severe left kidney and ureter pain that put me in the emergency room with suspected kidney stones. CT scans showed no stones, but damaged ureter and lots of blood clots expelled. A ureteral stent was placed on the left side and seemed to give me needed relief. Next day right side issues resulted in ER visit and eventual stenting on the right.

Oncologist gave me little explanation or support through the excruciatingly painful four-week ordeal and I had to rely on urology practice that also seemed to have inconclusive answers to my questions. Jevtana literature indicated up to 10% incidence of urinary problems. One of the urologists suggested it was a delayed radiation reaction, but my last SBRT was in fall of 2019.

So, have any others had ureteral issues with Jevtana/carboplatin?

I've had three rounds of both, Jevtana/ Carboplatin and a week after my last cycle I also seem to have an abnormal amount of blood in my urine. I have had a left ureter stent for over five years as a result of SBRT. (it's exchanged every three months) I will talk to my MO this Monday before my fourth cycle. I had a follow-up with my urologist yesterday and did mention the pain and Hematuria he suggested that it's possibly due to inflammation due to the recent stent exchange and being on Chemo. I will update as new information is available.

I've had many urinary stents (most on the left a few on the right) replaced every 3 months....I understand they now have stents that can be replaced every 6 months instead of 3. Reminder keep taking an antibiotic for each exchange.

John, my stents used to last longer, but in the last year or two 4 months seems to be the longest. About 5 years ago I had a stainless steel strny that lasted almost a year.. nut my current urologist doesn't use them.

He wants you to have them changed frequently so he can bill you often..........Many doctors are as dumb as a fox............New urologists may solve the issue.... if you wish to go that route.....but be careful (see my note below).

Note: without going into the gory details a replacement urologist* fucked up my sphincter forever. (*Her second stent change on me).........

Hey John, It's not my Dr. that's setting up the three month exchange program.. It's my body.. I've had a left kidney stent since 2015 or so... it was a present from my salvage radiation. Anyway back then I could go anywhere from 6 months to almost a year before I needed and exchange.. but as of the last two years or so, I get a terrible UTI that makes me totally useless to the point where I can hardly walk because I am so weak from the infection. So I arrange to have the procedure and in a few days I'm as good as new. As far as bladder incontinence goes, sadly I know that all to well, because of a fractured ego of my original urologist at Moffitt Cancer Center here in Tampa. After my third of fourth stent exchange, I went home with a catheter (that's how Dr. Pow Sang did it then). I could not pass any urine for about 1.5 days.. I went back and had to basically force him to attend to my situation.. anyway .. In the exam room he said "I will relieve the pressure by removing the catheter. I thought OK.. What I did not realize, was that he was NOT going to deflate the balloon, he just grabbed my penis and proceeded to quickly extract the catheter. His PA who is a 6.6 ft. tall retired Marine actually dropped to the floor and fainted. I gave out a scream and the exam room looked like a crime scene that I attended way to many times.. in my past life, with blood spray and spatter all over the walls and the ceiling.. due to the pressure built up in my bladder. If I had my weapon.. I probably would discharged in the direction of his groin. ..but that was not the case and I did not sue him either which in retrospect I should have. Because from that day forward my life as a functioning man was OVER.. Back tracking to 2011 after my robotic prostatectomy everything was back to normal within three months, no leakage, and full function of everything else. To this day Dr. Pow Sang (who BTW is the head of urology at Moffit ) can not look me in the eye when he sees me walking in the hospital. Are you aware of the Men's Liberty device. That's manufactured here in Largo Florida? It's a type of external catheter, but very different from the Texas cath's that are on the market.. truthfully without tat product I don't know how I would get through a day. The parent company is Bio Derm . lp.mensliberty.com/?utm_cam...

Hope this helps.... Go Blue! It's a prostate cancer thing
