1st Time posting. Need suggestions please.

My husband was diagnosed in Jul 2021 PSA 41 at the time, multiple mets in bones. n1b and m1b Gleason 4+3=7. I've been reading and trying to get as much info as possible from this website. Last Oct PSA shot up to 19. Xtandi started and he also gets the 3monthly Prostap injections. PSA went back down to 2.55 in May and has had a steady increase since then. 4.24 in Aug and 5.96 in Oct. Oncologist appointment on the 3rd of Nov. We're not sure what the next plan of action might be? His hip was quite compromised with the mets at diagnosis. Had been fairly pain free after chemo but he has started developing more pain in his hip. Any suggestions on perhaps next best treatment options would be appreciated. His mother had ovarian cancer, would it be worthwhile to get a BRCA test for optional therapies? Request a PSMA pet scan? Further CT or bone scans? Change from Xtandi? Any and all suggestions appreciated. Thank you.

Genetic panel is a must and it’s becoming standard practice. Have you looked into clinical trials?

Watching right now a live webcast for arx517 from Esmo 2023, a new molecule for mCRPC, check it out. Later I will check for the trial number. They are fast tracking to phase 3.

If it turns out your husband has a heritable BRCA mutation, you will want to make any potentially affected 1st degree relatives aware that they should be tested, too.

He should get both a germline (blood or saliva) and a somatic (tumor tissue) genomic test. If he is positive for a BRCA mutation, a PARP inhibitor may be beneficial. Pluvicto may be a good idea if he is PSMA-avid but not discordant on an FDG PET scan. If not available in the UK, he can get it in Germany or India.

Is the information from each of the tests different. For example, my husband had both tests. Germline did not show BRCA, but the genomic test did show BRCA2. Is BRCA2 worse if it’s Germline positive, or does it even matter.

Genomic "breakdown" occurs in metastases. So, in about half the cases, BRCA2 will not occur in germline, but will occur in somatic. PARP inhibitors are equally effective.

Are the Parp inhibitors started when/if he becomes mcrpc? Or should they start as soon as we got the test results back for BRCA ? He currently just takes Lupron, Zytiga and prednisone, and he’s getting SBRT once mapping is done. His PSA is .09 and testosterone undetected.

They are toxic. I don't know if there is any benefit in beginning to use them before there are significant numbers of DNA-mutated clones that would have been destroyed if BRCA were patent. There are a couple of clinical trials using them earlier, so we will soon know the answer:

Sometimes you have to take something with a grain of salt...... sometimes you need a whole shaker full. I suggest you view an old post labeled "Miracle Enzymes" from 4 years ago and make your own decision on the amount of salt you have to swallow.
