Just a quick update and looking for some advice if possible.

My husband Martin who has prostate cancer since 2019 and now has bone and liver mets has just completed 6 cycles of carbolatin /etoposide chemotherapy. The Tumours had shrunk from 18mm to 8mm after 3 cycles so we are hoping they will have shrunk even more now that the 6 cycles are completed.

We are also looking at the possibility of trying Pluvicto. Martin had a PSMA scan carried out last week and *” The PSMA scan showed uptake in the bones with no uptake in the liver or the lymph nodes. “. So it looks like the Pluvicto will work on the bones but what about the liver mets? I presume they do not show PSMA. Will Pluvicto have any impact on them?

Martin also had radiation to the lymph nodes in 2020 so does this mean they are cancer free now?

We are hoping Pluvicto will give him a better prognosis. Is there anything else we should be looking at? He does not have the BRCA gene so have been told Parp inhibitors won’t work.

If he has PSMA-avid metastases in his bones and non-psma-avid metastases in his liver, that is called "discordant." Please read this about treating discordant patients with Pluvicto:s

He may be better off with a BiTE trial.

I am also 70, diagnosed in 2019 with bone and liver mets. I had ablation of the liver mets by an interventional radiologist. It took about two hours as an outpatient procedure. Very easy and successful. PS the bone mets were treated with proton therapy.

my husband had Y90 treatment for liver mets by an interventional radiologist. minimal side effects. we won't know if successful for 2 mos. may ask about it. starts pluvicto in dec.

I have been following this very promising molecule for the future (first big cohort results will be presented today at ESMO), but the fact that liver expresses no PSA gives me some doubts. Anyway, look into this trial: annalsofoncology.org/articl...
