Hi to all, I was diagnosed with locally advanced prostate cancer back in 2018. I have had 40 sessions of radiotherapy and HT since then, I am currently taking Abiraterone with Steroids and I have a three-monthly De-Capetyl injection. My Gleason score is 7, (4+3). I recently had a Gallium PSMA PET scan at the London Clinic which showed Mets to my upper & lower spine and left and right ribs. I started Abiraterone in November 2022 and my recent monthly PSA readings have been 5.65, 5.37, 6.11, 5.67, 5.66, 5.64, 6.41, 6.96, and 8.20ng/ml while taking Abiraterone. My consultant informs me my PC is very aggressive and hard to deal with. He also told me he sees my type in about 1 in 100 men. I am now planning to have more CT scans to see where I am at. I think they will advise that I now do a course of chemo. (Docetaxel) Could anyone give me some advice on my outlook? Can a low PSA of 8.20 be a serious issue, especially the rate of rise over a short period? I am 63 in October of this year.
PS I was told by my GP when I was 57, I was too young to have a Prostate Cancer test!

It is not the specific PSA but the doubling time. Fast doubling indicates that your current treatment is not working. This is what alerted my MO to the fact that Abiraterone was no longer working. I did not qualify for LU177 because at that time you had to have had chemo to qualify. So I started on doxytaxel and have had 10 treatments. I should know fairly soon if it was effective. If not it will probably be LU177.
each one was different. the first couple I hardly had any side effects. them some really bad days for the next few and then again few side effects and the last two were pretty bad in terms of exhaustion. now after a few weeks I feel much better but still have the effects of virtually no testosterone since i have had an orchiectomy.
thanks, spencoid 2, looks like I will have to take a break from work! I will let you know how I get on. I will try and keep as active as possible. I am waiting for a CT and bone scan appointment, further consultation and then dates for my chemo. Looks like it will be in about 6 weeks. (NHS)
I'm on Abi + Orgovyx + 28 days of radiation for my small reoccurance in a single 5mm pelvic node. I don't know enought to compare the meds. Maybe Tall_Allen knows about Orgovyx?
I had APAC Biotech vaccines back about 6 1/2 years ago. You may have heard of NWBO that treats brain cancer. It is just about approved. It is the same treatment, but done in Delhi, India. The vaccines did not cure me, but I am at .13 PSA at almost 9 years out. I was at 212 PSA at 9 years ago so can't really complain. Some have been cured by APAC from what they tell me, but who knows. I know it's really rare to be cured from this disease, but it seems like it is getting closer. True dendritic cells like I got being made from your own tumor, not like the vaccines that are out there now that cost $93,000.00 that are out in the USA now. They really don't work like I'd like them to. Hopefully we will see this work for everyone soon. I think we are getting really close. I have been on Zytiga so who knows what that has done. I have only done ADT for a little under two years. The side effects about killed me. I know it helps a lot of people, but I just couldn't deal with the side effects. Vaccines seem to be the way.
