TURP Prior to Radiation: Opinions?

63yo, Gleason 4+4, PSA 0.055, 2 measurable mets in pelvic lymph nodes-right iliac chain

I finished a round of 6 chemo treatments in mid-February. I'm currently on Lupron and Nubeqa. My last PSA was 0.055. I decided to take a break before I start radiation in August or September to do some travel and recover from chemo.

I had a nice long meeting with my radiation oncologist just to get to know each other and let him know what my plans are. I believes that holding off on radiation till September isn't particularly problematic.

After chemo, I have significantly less retention in my bladder but I'm still taking .8 mg of Flomax nd my prostate is still moderately enlarged. All things considered, my problems with urination were pretty significantly reduced after chemo/Lupron/Nubeqa.

So, my dilemma is this, I hadn't completely given up on the idea of getting a TURP so that I can get rid of my urination symptoms without taking Flomax. But if I do decide that's what I want to do I have to have the procedure done before I begin radiation (EBRT). So here I am soliciting opinions and hoping that someone else has gone through something similar and can share their stories.

Thanks in advance for you time and your insights.

You'll probably want to wait at least 6 months after a TURP before starting radiation. Or you can let the hormone therapy shrink your prostate for a while longer and skip the TURP.

Thanks! I'm thinking I might skip it altogether since I've had pretty good results from the chemo/ADT so far but I wanted to see what other have experienced. I guess I don't like the idea of that procedure/option being closed off to me once I start radiation.

This article has info about turp before or after RT of the prostate.

It seems it is not a very good idea to have the procedure done, particularly if your urination problems are improving with the PC treatment and urination is under control using flomax.
