Low blood work prevents any treatment

I had Xtandi from July 2020 to December 2021 ,Stopped working and had chemotherapy 3 sessions wasn't working . I then had 6 sessions of Pluvicto .At the end of my treatment with Pluvicto February 8 my PSA was 301.

My blood counts as of of March26 was WBC 2.3 Hgb 8. 1 Plat 35 . They all have just started to go up from February 8 . Also on February 8 , I was put on Zytiga it didn't work .

On March 28 my PSA was 725. I was told to stop Zantiga. I was then told there was nothing she could do and that I would eventually need to go to a hospice and die. I am in good shape feel good no pain walk 2 1/2 to 3 1/2 mies a day. I am taking Procrit weekly increase my Hgb. The cancer is in my bones and lymph nodes.

I would appreciate any advice . I don't want to give up

Might ask about erleada.....i think you tried zytiga?....the other is reality sucks...if you didnt catch it early enough..and its deep rooted...the treatments....and or the search for may rob you of the quality time you have remaining.....might be time to prioritise...all of us in APC group will face it most likely.....your almot 17 yrs older than i......i would consider 75 a good run now....not saying " give up"...just saying ....live it up....bw

There is a "Hail Mary" option that you might try. That is BAT, where you are given high doses of testosterone to get your T levels to supraphysiological levels.

The high levels of T might kill cancer cells that have adapted to low-T conditions of ADT and other meds. The chances of this are perhaps only 10-30% depending on what mutations your PC cells have, but those odds are better than zero.

Additionally, the high-T should help with the anemia. It is also possible, if it successfully slows the cancer progression, that BAT could resensitize you to the meds that have already failed.

Many docs are not familiar with this therapy or the clinical trial that have validated its potential. Here is a good summary of it:

I'm only 68 and have moved onto BAT, self administered. As chance would have it, I am indeed a responder according to Dr. Denmeade who I met Jan. 12, 2023. My regular MO has agreed to monitor and assist with prescriptions. My MO at Dana Farber is also very curious and is monitoring as well. PSA lowered by greater than 50% on my second cycle.

Awesome, hope all works out. This subject is evolving rapidly. Many positive aspects of T has when used in conjunction with other therapies.

There's nothing in your profile. Have you had any metastases biopsied? Xofigo? Provenge? Jevtana?

Yes I had metastases biopsied at MSK . I have not had Xofigo or Provenge or Jevtana. Thank you for responding and would appreciate any advice you can give me.

Provenge may enhance your bloodwork so that you can tolerate more therapy.

Thank you. That should have seemed obvious but I usually only thought of its benefits singularly towards slowing cancer. That would be great to get better blood numbers as well for ability to get another treatment hard on blood numbers.

That's interesting about Provenge enhancing blood work. I'm considering Provenge and wondering if you could elaborate on that?

Have you had a bone marrow biopsy? You may well have myloidysplastic syndromes (MDS) with those blood numbers. Knowing might help in making treatment choices, but at our ages there isn't much we can do about MDS.

Hello brother. Its a tough situation that I suppose we will all face in time. Having said that, it is wonderful that you are feeling good and strong. That is the most important. If I were you, I would ignore the numbers, put my life in the hands of our Creator and enjoy life to the máx. You will probably live a long time.
