Prolia vs Reclast for bones

My MO has offered me a choice.

Prolia shot every 6 months vs Reclast infusion once / year.

Do both have the same potential for jaw problems?

It would be nice if you end up getting some evidence based confirmation of that proposition on the use of denosumab (XGEVA, Prolia) as a prophylactic against the spread of prostate cancer to the bone.

From what I've read, anything less than monthly is no better than nothing.

The risk of osteonecrosis of the jaw is not very different between denosumab and zoledronic acid.. The risk is low and ONJ usually happens in people who have inflammatory processes in the teeth because of infeccion or because they require tooth extraction or other surgical procedure.

Both are effective to control osteoporosis.

Zoledronic acid may affect the kidneys and it may cause an acute phase reaction with fever, fatigue, muscle pain etc, usually in the first 72 hours.

These drugs in the context of prostate cancer are used to treat osteoporosis in hormone sensitive cancer and to prevent adverse skeletal events, mainly pain and fractures in mCRPC.

I must say that is a real issue for me. Especially as I grow older I have more teeth issues.

Once you take this stuff, you are really at risk for many years if you need dental work.

You will be using one of those to combat the negative impact of the ADT on bones? are you osteopenic..hopefully not osteoporosis. Not actually used for reducing metastatic growth, is it?

My endocrinologist said that Reclast can be used for 10 years before switching.

I took Reclast in 2019 at age 54 and in 2020. My bone density improved enough in my 2022 done density scan (DEXA), that my next scan is in 2025 - 3 years.

I had Prolia shot every 6 months for 2 years while on Lupron, Zytiga, Prednizone. No bone loss. I had dental work done 2 years later, plus a root canal 2 1/2 year after last shot. Jaw OK.
