I'm on Casadex now going over 2 years. My PSA has been holding fluxuating between 18 and 22. just got my last blood test back from 1/31/23. PSA IS 32 now. I'm looking for the best way to proceed. Found out I had Prostate Cancer in 2017 or 2018 don't remember exactly. My next appointment with my Urologist is in April.

The first thing to do is to stop Casodex immediately. Over time, the cancer learns how to feed on Casodex, so stopping it may actually reduce your PSA. I was going to suggest seeing an oncologist rather than a urologist, but reading your profile, I see that would be futile. I hope you don't suffer with too much pain before the end.
I understand your hesitation with curative treatment options. However, if your cancer remains confined to the prostate gland, you still have a chance for a cure. I would recommend you not focus on the side effects of surgery and or radiation. Rather, take some time to read the many posts here of men with advanced disease for which surgery and radiation are no longer even offered and will eventually succumb to their disease. This disease becomes brutal as the cancer progresses. You have what many men lack, a chance for a cure. Just my two cents worth, but you would be foolish not to try.

You could move to zytiga. It is a newer ADT drug and stronger. It worked well for me after Casodex failed. The side effects will be similar to Casodex if any.

radiation treatment is pretty easy especially if you only need the seeds. Get to a good hospital. Won’t cost you a dime and get their plan at least. I agree the drugs suck but you may not even need them if they can radiate it. Waste no more time.
