High Blood Pressure and Secondary Therapy

I may need to supplement Lupron with one of the secondary drugs. I see that virtually all of them can cause high blood pressure, especially Zytiga, which is taken with a low dosage of Prednisone. I already have high blood pressure that is under really good control, without using a Beta Blocker.

I prefer Zytiga because it costs a LOT less than all of the other secondary drugs. For those who have taken Zytiga, I have a couple of questions:

1. Have any of you been able to switch using Prednisone to a diuretic or another drug that won't produce a rise in BP?

2. Have any of you been put on Beta Blockers to control your BP and how much success have you had?

My PSA on Lupron and Casodex went from <.0.04 to .04 (Quest doesn't go lower than .04) in October and I expect that in February (my next blood test) it is probably going to rise again. My PSA has been under control since 2016 so it could be my time using Lupron plus Casodex may be coming to an end and I am asking these questions as I know the secondary drugs may be right around the corner.

You have to take a corticosteroid with abiraterone to replace the cortisol that is inhibited.

It isn't enough to take BP medication, only a corticosteroid will suffice. If your BP goes up in spite of the prednisone, Inspra (eplerenone) may be a good choice.

Just because high BP is listed as a potential side effect, doesn't mean that you will get it from the drug.

You need to take the prednisone to avoid the increase in aldosterone (an hormone which increase the reabsorption of sodium and water and cause hypertension, edema and hypokalemia) cause by zytiga. I

f there is an increase of the blood pressure despite of the prednisone then they could add Inspra, a specific aldosterone receptor blocker which neutralize the effects of aldosterone. and it will bring the BP under control.

Eventually , zytiga may be taken without prednisone using high doses of inspra, but it is not the SOC.

I was on Zytiga for 2.5 years in combination with Lupron and Prednisone. After about 6 months I was able to eliminate the Prednisone. I had a stent installed in 2015 so was already on Blood Pressure Medication, but even today, I am at the lowest dose.

I was and still am on Lupron, in addition I took 250mg Zytiga with breakfast for 2 years with a low dose Prednisone. My BP did not go up. After 2 years the the Zytiga did not keep my PSA down, going up significantly from under 1 to over 3 in 2 Months. We stopped Zytiga and Prednisone and started Keytruda.

have been on Zytiga and Prednisone for years blood pressure never went up. Have been told not getting zytiga after Dec. going to be interesting what I get

why no more zytiga after December? Don’t want to go back on it until I have to but it worked when I needed it.

no reason, just a letter, my Dr. thinks it is because there cheaper products coming

I was also on Lupron and Casodex for about two years, then my PSA started to rise. My oncologist put me on a trial with Enzalutamide and an experimental drug. No high blood pressure 126/72, no side effects. My PSA is now 0.24. In July of 2021 my PSA was 0.57, in January of this year it had gone up to 1.21, by May of this year it was 2.81, that's when my oncologist got me enrolled in a clinical trial. The drugs are all covered by the trial. Tomorrow I go in for my blood work and get my Lupron shot.I was diagnosed with stage 4 and thought I was going to die, today I feel good, just get tired.
