any advice on Wht to do to help combat the weakness/ tiredness on Xtandi? Also is it as effective if you take less obviously while talking with the DR.

The only thing that can help is a heavy exercise regime. Of course, taking less is less effective - they did a Phase 1 study to determine the optimal dose.
Before I went on any ADT over a year ago I joined Anytime Fitness and I make going there 3x a week as important as taking the daily meds. After nearly a year having been on Firmagon,then switching to Orgovyx and now Xtandi added, I can honestly say that other than libido, and an occasional hot flash, I notice no other SE whatsoever... Xtandi added zero additional SE for me.
I do moderate strength training on nautilus machines to work upper body and strengthen bones and then I do 5 vigorous mins on the rowing machine....You don't need to buff out like Hulk Hogan, you need to just tone up and not allow muscle mass to atrophy or turn to fat ... I'm in the gym 15 mins 3x a week and I get precisely the results I had hoped for...
Also, I've read that worked muscles that are recovering require as much nutrients as the cancer cells so there could be a competition which is good especially if the muscles take priority... This is just what I've heard but what I know for sure is I feel energized, hopeful and happy and ADT or xtandi has not taken me down one iota . ....
Exercise is definitely the antidote. Both cardiovascular and resistance training.
Sleep is as important as exercise for me. Check your sleep quantity and quality. Much of the dizziness and lack of energy I attributed to ADT was actually just too little sleep.
Heavy exercise will help with sleep, no caffeine in the evening, don’t eat too late, etc.
On Erleada (apaludimide) and Provigil has helped me. It provides a slight amount of energy but mostly improvement in focus.
