My latest PSA test was 0.25 down from 0.30 i had 3 months ago. My Oncologist does not want me to start ADT yet. He wants me to continue monitoring my PSA so I'll have another test in 3 months.
My PSMA pet scan 3 months ago was negative and now this decrease in PSA leads to believe that I may have few Oligometastatic spots someplace. Any opinions?

i believe the rule is that therapies are changed or started when there is evidence of radiological progression of the cancer. Your last scan was negative for cancer, the PSA is stable around 0.3. Keep measuring the PSA and repeat the scan if there is a sustained increase of the PSA.
I’d recommend you keep monitoring your PSA, as your doctor recommends. If/when your PSA reaches 2.0, have a PSMA PET scan. I believe there is a 95% likelihood of finding the cancer with a PSA of at least 2.0.
In my case, following this approach recommended by MD Anderson, my PSA reached 3.7. When they did the PSMA PET scan they found a tiny spot in my left hip. Restarted ADT last October and they radiated my hip in February. PSA is back to undetectable again. Hoping to start my second ADT holiday in 2023. (I’ve been fighting this for over 7-years.)
Thanks James. My PSA has been rising ever so slightly since 2017 until it was 0.30 in May then went down to 0.25 yesterday that was a welcomed surprise since I have never had ADT
Read your bio, first thought was if we had to be in this group probably all of us wish we were in your shoes, your doing really good.Age 80, no ADT, low PSA, slow rise, so celibrate all that.
ENJOY your life, your likely to live to a very old age and natural death.
But get scanned regularly, until you find the cause of your PSA rise and then determine what action to take.
The harsh effects of ADT, chemo, etc treatments could be much worse than your PC. Take your time. Short courses (like 4 months) of Lupron could carry you years when and if you ever get to that point.
