I am still on Zolodex injections every 3 months not working PSA rose from 100 to 190 over last three months currently this is only treatment given having exhausted zitiga and Xtandi and not too keen on pursuing chemo at 87. I have heard some have had good response to relugolix which is unfortunately under review currently by Health Canada. Can I get this from the US and what would be the cost. Recomendations....probably cant hurt

After castration resistance occurs, it won’t matter if you switch to orgovyx. Chemo is underutilized unfortunately. It’s use depends on your health status, not just your chronological age.
Consult about having chemo with a lower dose and every 2 weeks. It has similar results and it may have less adverse events.

Just compromise to one treatment, see what happens and make a decision to have another treatment. You do not need to compromise to all the treatments used with chemo, just to one and then decide if you want to continue.
You can try Firmagon monthly injections instead of relugolix. It was recommended by one German internet site to try after Zoladex fails.
Can you ask urgently for a liquid biopsy?
Can you try reduced dose of Enzalutamide?
Or at least can you try simply bicalutamide 50mg per day. It may work for a short time. When bicalutamide fails, it will fuel the cancer grow so you should stop it.
Similarly on Zolodex, but given recent events do not know if that will continue. Now Xtandi stopped, and have done some radiation in the last week to address pain on the left flank and back, after bone scan and CT scan showed additional spread. Liver now engaged and Spine has more cancer. After this chemo will be starting. Xtandi worked well- 3 pills rather than 4 for one year and 9 months. Three pills as 4 was too potent and resulting in a fall that first day. Physisican was comfortable with 3 pills, to address risk of falling, No other side effects. Psa recently rising but still less than 2.0. Hoping radiaition works to reduce pain and chemo can clear multiple small liver lesions less than 10 mm, and curtail the growth in the spine. At 71 a bit younger, but chemo just might help. I think it is less about age, but about the progression and the science behind what seems to have worked for others.
