The only meaningful absolute figure would require a Bone Alkaline Phosphatase (BAP). The usually measured ALP comprises a mixture. There is a very crude rule of thumb claiming that when bone metastases have set solid foot, BAP ~50% of ALP. If you have very old ALP readings, the older the better, you can compare them with current and see if it has gone up and in such a case by how much.
i found an old ALP from 10 years ago at 48 and compared it to a Bone specific ALk Phos I had done very recently of 15.8 so that makes me feel ok I guess. Your reasoning on using an old reading is because ALP goes up over time? My general ALP is 65 now, so yes the ratio would be lower.

My reasoning is that older ALP tests bear stronger credentials of bone metastases freedom. It is based on the macabre logic that in case there were bone metastases then, posing such a question today would had been very improbable.
In general for PCa, an increase in ALP is associated with bone turnover and can be caused by increase in metastatic activity, especially when accompanied with rising PSA. There are other sources of Alkaline Phosphatase though so it's not an absolute measurement. Best to use imaging for a determinatuon of progression.
