Alkaline Phosphatase Rising

Hello folks, Stage 4 with bone Mets and lymph nodes. Current PSA at 0.008 and i was diagnosed in Nov 2019 with a PSA of 308. I’m looking for answers as to what is causing my Alkaline Phosphatase to continue to rise. Currently at 161. My bilirubin is within normal range which suggests its rise is not being caused by my liver. My oncologist says its my bones causing the increase. My hemoglobin, hematocrit and MCH are all low. Last bone scan showed bone Mets were reduced with some no longer visible. Lymph nodes have returned to normal size.

Bone marrow issue?

Any help would be appreciated. Thank you…

You can get a bone-specific ALP test. Your PCa may not be putting out PSA. Discuss getting an FDG PET scan.

Prior to starting my tE2 (transdermal estradiol) gel regimen my ALP was well above the normal high level which was an indication that something abnormal was going on in my bones. You have been on Eligard which is chemically castrating you. Estradiol is necessary for bone maintenance and it is aromatized from testosterone of which you have little to none. Shortly after applying the E2 gel my ALP began dropping to well within normal levels and has remained in the low 80s for over three years. I am hormonally (as opposed to chemically) castrated from the gel which is a more natural; however, unconvential way to shut down the production of T. This regimen requires no doctor visits, no injections, and has minimal side effects...my only SE is cute little boobies!

Ron, this is such a good method of ADT. Why is it that my MO s don't use this more commonly?

Instead we have to fight a disease that's deteriorating our bodies, and a treatment that deteriorates our souls.

I really think that money is a big factor in this lack of approval in America. I spend less than $20/month on the gel and haven't been to a doctor in over four years. From what I understand the nasty Lupron injections are about $6000 every three months. Richard Wassersug (the tE2 guru) has been using the gel as mono therapy for about 20 years and still has an undetectable PSA. He as I, had an RP and experienced BCF.

Same here in OZ, nobody listens when talk of tE2 comes up. It just has not been approved here & probably never will be because cancer is Big Business!

You could discuss getting a DEXA scan, Bone reabsorption because of ADT and the anti androgens could elevate the ALP. Depending on your clinical situation treatment with bone agents may help to increase bone density and it may decrease the ALP.

You need to be on Denusomab to bring AP to normal levels. Ask your MO for advice on this and bone scan.

Dale, read your Bio. Have you had removal of your prostate by RRP or RT?Most cannot get to your low numbers without.

Good numbers despite. I like what I read about tE2 patches above. Ties in with PJ OHeas’ response Dr Klotz video.

My ALP went up and up for about 6 mo. followed by PSA trending up... when PSA started to sky rocket normal scans still showed nothing. Na-F18 showed 2 new bone mets. They were Zapped followed by Provenge. PSA dropped and is still dropping.. ALP is the lowest it has been in over 3 yrs., so something is doing its job. ALP-55, PSA -0.26 as of yesterday... Life Is Good , especially when your numbers are all going in the right direction....

Thanks Shooter1 for the info. Right now I’m monitoring the ALP increase. Oncologist feels that its nothing of great concern as my PSA is so low and my liver tests are good. Wait and see game I guess. Take care…
