Treatment post ADT failure?

Reaching out to fellow warriors, yet again, for advice. It has been very helpful in the past.

After a little over 2-years ADT has failed for me since Jan 2022. Cancer is spreading fast - PSMA PET scan found 8 new spots in my bones. The pain gets a bit worse every day in my ribs and pelvis.

My oncologist has talked about a few options - Apalutamide, immunotherapy, chemotherapy. LU-177 is only available to me in a randomized trial (50% get a placebo). As I understand, from all the available options for us LU-177 has shown the best results. I know some of you have done LU-177 already, can someone please give me a cost estimate for the treatment, I want to see if I can afford it.

What do I do now? Maybe try chemo and if that does not work, at least LU-177 will be an option then?

With fast spreading Gleason 9, it seems my time may be running out! While I have made some plans for my 13-year-old son (single dad), I would like to further solidify my son's position post me. I know it is not a fair question but does anyone want to give me a guess on how much time I may have to wind up my affairs? I know I should be ready every day but I still have to work to put bread on the table.

If you have bone and LN metastases only (but no visceral metastases), actually Xofigo does slightly better at improving survival compared to Pluvicto.

Or you can spend 15 weeks doing 6 Taxotere infusions, and then qualify for Pluvicto. Either way,

Provenge would be a good starting point.

Just ADT failure.. Stay on it and get chemo.. you, like me, should have a few more years with that. I am now on Xofigo with no side effects noted. I've burned through most available treatments in the last 5 yrs but still have he RO options to hold the aPca back for a while longer.. Best of luck to you. Hope you get to see your son graduate HS and move on in life while you are here as a mentor....

Thanks shooter, that is the goal, to get him to 18 ;) BTW I used to be called โ€œshooterโ€ in college, I was on the rifle shooting team for NCAA matches ๐Ÿ˜Š

I shot competitively in national matchs with the NMLRA and placed well for many years. In 2017 I was so weak I couldn't even raise my 32 cal squirrel rifle, but was back at it in 2019. Shoot the worst scores I ever had in the week long comp. but though exhausted still had a good time. Life Is Good, so is an enjoyable hobby.

Nice! I went to camp Perry too for the national matches a couple of times, won a few medals ๐Ÿ˜Š it is a fun sport, I need to go to the range soon ๐Ÿ˜Š

I shot expert In boot camp , USMC , 1970 , it paid off when serving in Viet-Nam . M-14 , iron sights .

Erleada (apaludamide) is a fine product. Only side effect I've noticed is some fatigue. Best of luck.

Barry , you canโ€™t give in . Your son needs you around . 13 is a critical age for a boy . I Know that I went a bit wild at that age . You have options it appears . Shooter has put It down again . Iโ€™m pulling for you to knock it back into the dust again . Doctors donโ€™t always take into account our will and our strong reasons to live . Youโ€™ve got to put that pain away . ๐Ÿ™๐Ÿ™๐Ÿ™๐ŸฅŠ

57 years old and a single DAD with a 13 year old son. If that ain't a kick in the balls.... I want to cry but hopefully things work out for you and you see him get his doctorate degree. May God shine his light on you and your son and if gets his doctorate I'll take him for dinner in the best restaurant in your city/state!!! God Bless...

Thanks John! He will appreciate that ;). He is quite a cook and enjoys a rib eye ๐Ÿ˜Š I moved up things a bit today and took him out to teach him driving, I cannot afford to wait until it gets โ€œlegalโ€, he did good ๐Ÿ˜Š

Good for him..... I taught at least a dozen people how to drive. It can be very daunting.....For teaching I used to use the NY Mets Parking lot in the off season since no cars where there. Anyway tell him three Rib eyes waiting. One for him, one for you and of course me....p.s. When I taught my son how to drive in the city streets I told him I would yell out STOP and he was to hit the brake. This was to make sure he would obey my command and stop on a dime (scared the shit out of him many times)...
