CMP and PSA update

After 100 day on Lupron and 45 days on Abiraterone my PSA is 0.25

my CMP is normal except for AST which is 39 on a normal range of 15-37

ALT is 56 on a 15-61 normal range.

PSA is clearly a good #, should I be worried about rise in AST and ALT as they have both doubled since starting these meds. ? for the previous 10 years they were both in the teens.

Sounds like my liver is getting stressed.

Sometimes they come down if you cut the dose, and then ramp back up. I guess the liver acclimates to it.

How many tests have you had since staring Abi? Have they trended up?

Been on ABI 6 weeks had first test 30 days ago AST was 29 a month ago up from 16 a month before that.

AST and ALT are liver #'s. Liver issues are known with Abi ( I assume you know that which is why your asking) but let your Dr tell you if they are worthy of concern. In range is always good and they could drop on your next set of tests. If they go up again and your Dr says nothing I would be make sure he's watching.

The #'s I have seen for a bad reaction are still much higher than yours.

Ask your MO. Elevated and high are two different things. My AST & ALT were elevated for weeks after starting Erleada but eventually came back down. Now they’re both in the teens.

That looks like a trend to me, in a short period. I had small jumps and then they dropped, but then I was on 750mg daily after my issue with WBC's in Feb 2020 when I stopped for a few weeks.

I noticed my ALP went from from 67 to 90 since February. Since the 67 I have undergone radiation in several areas and ADT for 9 months. Seems like ALP is the one you would be more concerned with. Of course, they all matter.

He means if you are thinking it’s possible you might cure oligometastatic cancer, the PSA at the first 100 days after completion of RT are significant

Because you see if the treatment are pushing down the ol Psa or not! Some guys aren’t so lucky . You are .. a declining Psa is you friend now . Keep it rolling .. 👏👏👏🏋🏽‍♂️🥳

I have been on abiraterone for 3.5 years. ALT and AST have off and on been slightly elevated which is not unusual. My question is if you have had a PET/CT yet. I say this because of my personal experience which pointed out the importance of PET/CT which was re-enforced by watching Dr. Eugene Kwon's recent youtube videos (2 parts) on a DIY Combat Manual for Beating Prostate Cancer. He stressed if you have not be PET scanned you have not been adequately imaged. Would strongly advocate all patients with prostate cancer and physicians taking care of patients with prostate cancer watch these two videos. Well worth and hour of your time.

thanks slpMy MO also agreed to order a PSMA PET Scan, waiting now to see if medicare will approve it.

I did have an Acumen PET scan in August so not sure they are going to go along with it. Then I will have to decide if it warrants self pay if not covered.

Your ALT and AST are allowed to go 5 times higher than normal. That is where my numbers went in about month 7 of zytiga. I stopped completely to allow return to normal. Restarted at 1/2 dose and ALT & AST were ok. Went to 3/4 dose and have stayed there. All PSA at <0.01. Just completed 4 years of lupron + zytiga.

And once mine got 2 times normal, my MO ordered weekly blood tests to monitor.
