Hello, my father cancer spreading to his bones . Part of him wants to give up but a part still has some fight in him ! Had radiation therapy when there were only 4 spots but now there are spreading all over his bones . Wondered about the luthetium 177 therapy in Germany or Austria . ( arrangements, costs Etc ) Merry Christmas and God ‘s blessings in your every day fight !

Xofigo is already approved and available in the US for painful bone metastases.

Thank you ! My dad decided to move out of states and is determined not to come back so that is not an option. We are 4 siblings and we all rotate staying with him. Overall he is still very active and with a very good sense of humor .

Lu 177 PSMA will treat metastases anywhere (bone, lymph nodes and organs). Xofigo will treat only bone metastases.

There are several trials for Lu 177 PSMA and Ac 225 in the USA that are recruiting:

He may qualify for one or more of these trials. Ac 225 combined with Lu 177 PSMA may be better choice if he has too many bone metastases.

This is the best choice of therapy for today according to the advanced protocol "177Lu + 225Ac - PSMA-617"! I have completed three courses of therapy according to this protocol. The result is excellent! PSA dropped from 212.9 to 0.140, bone metastases disappeared by 95%!

I was treated with Lutetium and Actinium isotopes in Baku, Azerbaijan. I think that today it is the best choice according to the criterion of "price - quality"! I have already written a little more in the comments below...

What can be a secret for someone who writes in Russian..?!)) PSMA therapy works on all cells that have the same PSMA receptors, no matter where exactly they are located! This carrier molecule (ligand) has many places of physiological accumulation: salivary and lacrimal glands, prostate gland, adrenal glands and others. The difference is that prostate cancer cells have 200 times more expression of these receptors than healthy cells! It is this fact that destroys them when PSMA molecules, "charged" with therapeutic isotopes 177Lu or 225Ac, attach to their surface - then the total radiation dose is 200 times higher than that with which healthy cells are irradiated and eventually kills them! Personally, I confirmed with instrumental studies that I no longer have a primary focus in the prostate gland itself, other soft tissues, and my MTS in the bones have disappeared by 95%! I have conducted 4 courses of such therapy with this "cocktail", and my PSA has decreased from 213 to 0.140 and continues to fall.. Here is the dynamics of PET-CT after three courses of therapy:

My husband had two round of Lu-177 and PSA has decreased. He is going into his 24th year of battle with PC. His oncologist told him that next year, Lu-177 will be available at many US hospitals. Currently, he goes to Lu-117 at UCLA. He discontinued the rounds since the first two did well. Also, he had his port removed due to complications of infection and is still fighting that since July. Weird how that almost did him in rather than the CA or Lu-177. On with the fight. If you have any other questions, please ask. Best of luck to you.

My father did an actinium trial at Cornell. PSA dropped at first but it now back up higher and he is incredibly frail. Talking about hospice now :( Glad your father had a good response!

I am also on a trial at Cornell with Dr Tagawa using Actinium223-J591 + Xtandi/Enzalutamide + Keytruda for the past 18 weeks so far. Having my next appointment in 2 days with my next 6 week Keytruda infusion. My last visit showed PSA rising.

What was the treatment regimen on your father's trial and how long was he on it before he stopped? What else can you share, especially side effects? Has he started any new treatment yet?

Hoping for the best, his first two treatments he didn't really have side effects. I little dry mouth and fatigue but nothing crazy, after his last treatment, his platelets dropped below 10k and he couldn't receive his last. His PSA was high...1000 to begin with. Dropped to 500 and then went right back up and is at 1200 currently. He has stopped treatment like I said. His was actinium223-j591 only and no other meds.

I went to Prof. Dr. Baum in Wiesbaden, close to Frankfurt. Everything. PSMA PET scan, treatment, subsequent scans, 3 days in the hospital, consultations are around $15K. Prices may have changed and the dollar is stronger now than then. Highly recommend.

Hi Jasman, how advanced was your case before Lutetium? How many rounds did you do ? 15 K was total cost for whole treatment? Sorry could not find your detailed post.I am on vacation after second ADT RP January 2018 went in PSA 34 G9, clean edges but after a few months undetectable out of the first ADT it started showing doubling time 3 months

I will do PET PSMA in January and depending on what kind of findings will decide on either precission shooting if they are few or go for something else, it would be an early Lutetitum mi PSA is expected to be around .4/.5 in late January

Luckily, I only had soft tissue metastases. I didn't have surgery as good doctor friends recommended against it. I'm glad I didn't now. There's so much happening with less invasive treatments, such as the BioNtech mRNA transport, DS7300a, LU-177 and its variations, etc. I am Gleason 9 and my PSA peaked in the 50s before I made a move. My last scan showed no mets and a greatly reduced primary. I'm currently sniffing at the DS7300 as a possible next move when my uninvited friend decides to become more aggressive. Always have to have an ace in your back pocket. I spend many hours looking at what's out there and check the oncological symposiums to see what's being presented. You sound like you're in good shape. Just keep informed and check everything out very well before jumping into any treatments. Be careful with the LU-177 treatment as it is only as good as the oncologist administering it to you. It's not as easy as it sounds. Dosing and various mixtures require experience, and, requires a very good CT scanner that's up to date for the full picture. I'm more than happy to discuss any of this with further, if you wish. Stay positive and tell yourself every chance you have that you're going to make. And you will.

Jasmann Thanks for the advice. I am fighting it and apparently its pretty much controlled still, but I had a gap last year had to have open herat surgery for aortic valve replacement and it got pretty nasty since I have un undetermined bleeding disorder that only surfaces in major surgery , got me off my horse for a good long 7 or 8 months and my second round of ADT was delayed thus so I am going back to Argentina and might have the PET CT PSMA done there or maybe wait for Dr Baum, but nuclear medicine in Argentina is pretty well advanced, we've been doing PET CT PSMA for the las 6 years, so we have good experts and the scanners are digital which is important. I am residing in Prague for the las 4 years, but still have my trusted doctors in Argentina.

Anyway the numbers you mentioned about Dr Baum seemed very reasonable, do not know how many cycles of Lutetium was that.

I off any medications right now and back at my best tennis in the last 10 years so I guess I'll just keep going. SO far you've only done lutetium then?

Look forward to hear more form you, sounds very interesting and again thanks fpor sharing. I am definitely in for the fight.

Happy to hear you are on the mend. Prague has some excellent medicine. My good cardiologist buddy went there for his ablation. You're a stone's throw away from Baum. The number I quoted was for one cycle, but not every cycle requires the PSMA scan. That's around $4k itself. The treatment is relatively inexpensive in comparison to some chemo. I was on Casodex for a while, but that was all the conventional I did. I never had any major side effects so feel very lucky. But, it's a full time job keeping it at bay. Keep in touch and keep the fighting spirit. I feel it's as important as the medical treatments.

Good day! I apologize for my English, I am writing through a translator. I want to tell you about my personal experience of treatment according to the advanced protocol "177Lu + 225Ac - PSMA-617" in Baku Azerbaijan. The result is amazing, after three courses of mts therapy, 95% went away in the bones, the primary focus in the prostate and soft tissues was not reliably detected on PET-CT! PSA dropped from 212.9 to 0.14 This nuclear medicine center is an active member of the European Association of Nuclear Medicine, which guarantees the quality and safety of the treatment! The cost of treatment is twice cheaper than in Europe because the purchase of radioisotopes of Lutetium and Actinium is subsidized by the state! In other words, for the same money, you can do two courses of therapy than in Europe! If there is an interest in conducting treatment in Baku, I will help with logistics and arrange direct communication with the head of this poison center, Dr. Fuad Novruzov! I believe that people with our diagnosis should help each other regardless of the country of residence, and therefore my help is completely selfless! And God bless us all!

Of the side effects, this is, of course, xerostomia and damage to the lacrimal glands - these are the places where PSMA accumulates physiologically, despite all the manipulations with cooling these places during the therapy itself..((But it's better to be toothless and blind here than with teeth and eyes there in heaven..)) Yesterday I had an operation to replace the lens in my right eye, and now I have a new eye at 55!)) After the New Year I plan to make prostheses and I will have new teeth!))

I just don't know how to do it better, so as not to advertise the tools and products that I personally use and that help me a lot!

Thank you . My dad last PSA was 175 and multiple bone metastasis . He had palliative radiation when he had 3 bone metastasis but now there are spreed all over his bones but no other organs. I would appreciate any help regarding logistics ,contacts , hotel recommendation etc
