Hello everyone. Id like to post update on my fathers therapy. This time last year he got the therapy of Zytiga + prednisone 5mg a day. Before the therapy he was in poor health. He was diagnosed with MCRPC. If you read my previous post you can find out that he has multiple methastasis through out his skeleton and having pain most of the days. PSA at the time was 750.
After taking the therapy he didn't have any major pain, or other difficulties. His quality of life has improved by a lot. Now, he did last scans and showed few more spots on the CT scan, and MRI shows no progression. PSA is going down last PSA is 54mg. Thank you for the support the last time I wrote on this forum. I take my 4 Zytiga a couple of hours after dinner, just before bedtime. My thinking is the tiredness that many experience is lost in 8 hrs of sleep. I take my 5mg of Prednisone in the morning as it gives energy. Works for me anyway. Btw: my PSA was slow to fall (many get to less than 1 in 3-6 months. My PSA fell from 47 to .67 but it took 11 months. My prostate is inoperable, so still in place.
Not 750 like your Father. Some have said slow to Nadir is better, as slower to Resistance. I think all of us are individual and respond independently to therapy.
Xtandi failed me in 1st & 2nd month with T rising from zero to teens, then 20’s. I switched in 3rd month to Zytiga (which many said will probably also fail), and it immediately brought PSA & T down each month.

My father also has orhiectomy and his T levels were always low, but Zyiga helps as LearnAll sugested in the post below with "blocking T from all 3 sources ( testicles, Adrenal glands and Cancer cells themselvs".
Lupron blocks Testosterone only from Testicles whereas Zytiga blocks T from all 3 sources..viz. Testicles, Adrenal Glands and Cancer cells themselves. In this way, Zytiga is broad spectrum drug. However, Zytiga also causes increase in blood pressure and Increase in blood sugar more frequently than Lupron.
That's a great question ! Its the SOC masters who make the rules. Sometimes..contrary to the common sense and logic.I have started Zytiga Monotherapy 2 weeks ago. (Casodex started to lose effect after about a year) My PSA in 2 weeks with Zytiga Monotherapy (250 mg with high fat dinner)
has come down from 7.1 to 5.4 and it keeps falling. I was able to convince my Onc for this experimental regimen . If this succeeds ,I will write a full post about this experiment in about 6 to 8 weeks.
Remember..the true success of any treatment is determined by how low PSA falls..it should reach at least 1.0 in order to call it success.(in prostate intact men)
