On Zytiga past 38 months. Great results. Now seeing a bump from .03 to .10 to today’s .25. Wondering how high they let it go before switching to next miracle drug?

Your PSA may be rising, but it's still very low.

It's not recommended to change treatments based on PSA alone so I would start discussing when next to do imaging in order to see if there is progression. Then if/when there is progression, make a decision about the next treatment. That might still be quite a bit down the road for you.

Perhaps a switch from prednisone to dexamethasone is in order. That works for a decent percentage of guys.

I am a over a year out from my change to Dexamthesone so it worked for me. I realize that's only one data point.

PCWG3 guidelines for clinical trials is that 1.0 ng/mL is the minimal starting value if confirmed rise is only indication of progression.

At 0.5 and up PSMA scans are useful, at 2.0 and up Axumin scans become useful. Once your PSA hits a number where a scan is viable you can use a scan to see what is going on, perhaps it’s something you can address with radiation in order to buy time before moving on to the next treatment.You’re also at a point where Provenge is most effective while your PSA is very low. I’m in the same boat, after several years my undetectable PSA is now detectable, although still very low. I recently completed Provenge treatment and will review my status with Dr. Sartor next month. What I outlined above is the game plan he has laid out for me, thought I’d share, good luck. Let’s keep kicking that can down the road.

I have a stupid question. If one's prostate is still intact, can ADT still lower PSA to almost zero? Or will a prostate always generate some PSA.

Doesnt hurt to add sulforaphane (Broq brand in US) to your protocol. Read this clinical study. pubmed.ncbi.nlm.nih.gov/259...

Those tests--3 mo apart/?? If so still pretty good. My PSADT went to 12 days before Na-18 scans showed new mets==since zapped. Normal bone and ct scans were clear.
