Throwing this out there even tho it may have been addressed before, so don’t be mad if it’s a repeat question, just ignore then. We finally received the first bottle of Xtandi yesterday but my husband is still hesitant about taking it. He has a lot of other health issues beside the dreaded recurrent prostate cancer, such as 3a kidney disease, Diabetes, high blood pressure and is really worried about the effects of Xtandi. Yes, trying to treat the cancer and prevent it from spreading is a far better solution than the side effects but he’s still really, really worried. I told him that I would be willing to discuss his concerns but I cannot make the decision to start the Xtandi for him. We don’t see the Oncologist for two weeks. We see his nephrologist in about week and a half and cardiologist next week. Some of the meds he’s on from those docs had side effects with the Xtandi as well. Stuck between rock and hard place I guess.

Try it and if you have any trouble cut the dose. I had to cut mine to two pills (80 mg) a day. Full dose was to much for my body to handle... Good luck to you...
Granted some people have bad side effects with xtandi but he won't know till he tries it. He can always quit. His doctor should be testing his liver function.
The "phamacokinetics" of Xtandi typically does not result in big changes on the timeframe of days or a week or less. whether starting with the regular (4 capsules) dose or 1/2 dose. The elimination half-life of a typical dose is nearly 6 days. It builds up in the blood to a steady state level of around 8 times the daily dose within the first month or so. BTW, most elimination of Xtandi is done via the liver and not the kidneys.

Xtandi worked for me for about a year, then psa started creeping up again.... have been on zytiga for about 6 months and psa has dropped from 19 to 14.5..... also, the lesions on lymph nodes have not increased in area during that period. I have almost no side effects other than getting tired when I play golf, which I still do at least twice a week! i'll be 81 in September was diagnosed in 2010 and count my blessings every day. My wife and I (she had colon cancer) hit happy hour at our favorite bar every week and we're headed to Florida in October and San Diego in November..... It's probably going to get me eventually, but I've been blessed and you must keep fighting and asking questions. As Jim Valvano said years ago, never never give up!!
