Hello, I really appreciate your continuous sharing and supports since I joint this forum. We have many worries that we don't know who to ask, so we'll bother you again. The question is:
My father should keep using 1000 mg zytiga + 0.5mg dexamethasone or switch from 0.5mg Dex to 30mg hydrocortisone.
As said in my previous post, on June 28, 2021, following the doctor’s indication, my dad took a test of cortisol level at 7am and the result’s 1.5nmol/L (the lab’s threshold’s 133-537nmol/L.). His doctor asked him take Hydrocortisone 30mg/day (20mg in the morning and 10mg in the afternoon,) instead of Prednisone 10mg prescribed before. The doctor did not explain the reason of this switch and we saw limited sharing of using Zytiga + Hydrocortisone. We have searched, received sharings (e.g. from Tall_Allen, GP24) and we understand:
- 30mg of Hydrocotisone in the combo Zytiga+ Hydro has the effect of compensating the amount of cortisol, which should be produced by adrenals. This amount of Hydrocotisone cannot be used to treat Addison's disease (cortisol insufficiency) when using zytiga
- Looking at the table of comparison of systemic steroids (dermnetnz.org/topics/system... , that using 0.5mg Dex is likely better than 30mg hydrocortisone, especially about biological half-life (duration on action) and causing HPA (hypothalamic-pituitary-adrenal) axis suspension.
Is my understanding regarding 2 points above correct or not?
Because of my current understanding, we want to continue with Zytiga+Dex. However, the failure to comply with the doctor's instructions made us concerned.
