Prednisone switch to Dexamethasone with Zytiga

If I switch to Dexamethasone do I need to wean myself off of prednisone first or can I stop Prednisone one day and start Dexamethasone the next day with out any repercussions?

No. A straight switch will be fine.

Here is a dose equivalent calculator: mdcalc.com/calc/2040/steroi.... If you switch to equivalent dose- no problem.

5mg P = .8 mg Dex

what it did not say was how much less frequently I could take it based on longer half life of Dex

People typically take 0.5mg of Dex when switching from 5mg Pred

It was last year, I believe it was 4. Unfortunately I did not respond to the change. I went forward with a Trail Study at Dana Farber LuPSMA177.

sorry to hijack this thread but the docs are talking about me doing the switch idc , does it work and are the new side effects as common and the meds indicate they are ?

what was your low lowest PSA? Current PSA?

PSA dropped to 0.11 from 342 on diagnosis . It stayed at 0-11 from 2017 until 9 2022 when it increased to 0.28 in 1 month . Then dropped to 0.26 then 0.24 and docs wanted me to swap to dexa from pred. Next month was 0.22 and decided to not swap after all . Then 0.22 again and this week 0.12 .

So for now I remain on pred but I asked the question that was in my mind a few months back as I am sure I will be back there again .

Hi, Irun. It seems that the switch works in a bit more than half of men:

It worked for me for about 18 months until PSA progression.

I stopped pred one day and next day on dexa. No difference.

Is there a reason you are switching?

Why are you against use of SARMs?

why do you assume that a drug designed to bind to the AR receptor, won't bind to the pca AR receptor and feed the pca?

Good question indeed but way beyond my pay grade, none the less I had decided to cut out SARMs for the time being.

plan is to retest in 2 weeks and decide about switch to Dex. SARMs will have cleared by then but why not just move on to Dex anyway?

Do you see any downside to moving to Dex?
