Its true that small cell prostate cancer is an aggressive variant. But..without full details..such as biopsy report, PSA, ALP, Scan results, type of bone corrosion and other test results..it is very hard for anyone to provide useful guidance. About 2 to 3 % of all prostate cancer come under small cell category. Not so rare.
It is rare, but a few men on this site have been diagnosed with it. See this recent post, for example:

Because of the pancreas involvement, you might want to explore somatostatin or Lutathera if IHC of a tumor shows that somatostatin is expressed.
I recommend that you go for a second opinion from Misha Beltran at Dana Farber. She is as close to a world expert in it as there is.
This is also called neuroendocrine tumor. Standard treatment is Chemo with docetaxel and carboplatin.
However, I would try to apply 177Lu-DOTATATE therapy: pubmed.ncbi.nlm.nih.gov/315... This therapy is approved by the FDA for some neuroendocrine tumors: ascopost.com/News/58482
I had a small cell component to my PC - threw the kitchen sink at it: ADT, pelvic radiation (35 days) and chemo at the same time (etoposide and cisplatin). It knocked me down but not out, that was five years ago. The small cell component was gone after that initial round
A friend of mine has it and just completed an intensive program of chemo, radiation, scans etc and is now in remission after about 9 months since diagnosis. He used his local MO and RO for the treatments but Tall_Allen on this website recommended to me Dr.Misha Beltran in Boston as a specialist with extensive experience in small cell carcinoma, so my friend contacted her and she took over the management of his case remotely from Boston and he has been delirious with joy about the results so far. He is 57 years old. I recommend you call Dr. Beltran's office at the Dana Farber Cancer Inst in Boston and discuss your situation with her. Good luck!
