After 3 months on Zytiga and Prednisone with no drop in a climbing PSA ( 6.4 in 12/19 to 20.1 on 11/14/20)we elected to try Dexamethasone with the Zytiga. I was on Xtandi for 3 yrs prior. But I noticed my evening bladder leakage problems got much worse. I usually just use a Maximum Depends Guard at night but all of a sudden I need more. I try to cut way down on liquid consumption after 6:00pm but that doesn’t seem to help.Has anyone else had this problem?
Also it looks like I am definitely ready for a new approach, maybe Radium 223 or straight to Chemo treatments.

Xtandi failed after 3 years and Zytiga is failing sooner. If this continues and no improvement, may be it is time to think about Docetaxyl chemo... Lets see what happens with Dexamethasone booster..
Thank you, I have no pain that seems to bother me and I am trying to be patient with Zytiga, but I feel It is about time to take the jump Docetaxyl. I might add I’ve been on Eligard and Xgeva since 2014. And I also have had Provenge treatment.
Why I’m having more problems with the bladder I don’t understand. I was taking 10 mg of Melatonin but stopped that thinking I was sleeping to sound.

Urine analysis needed , you can do it with dipsticks available over the counter. If you see abnormal Urinalysis showing urinary infection.. get it treated promptly by antibiotics and Caneberry plus D- mannose. Not a prescription but a suggestion to discuss with doctor.
Never had a problem when taking prednisone but when I switched to dexamethasone I started having leakage . I now wear panty shields and they are the perfect solution.
Different drugs apparently affect continence differently in people. Prednisone made my incontinence significantly worse. I only took dexamethasone with chemo, along with prednisone, so it could have had that affect too.
Well the beat idea may be to go straight to chemo such as docetaxel for a course. In addition to hitting the cancer back it might restore some effectiveness for Xanti for some more time. Get your blood sugar and other chemistry checked as well as urine analysis done. Corticosteroids such as prednisone and dexamethasone can raise blood sugar to the point of spilling into the urine causing
Much increased urine volume. Especially if you are pre diabetic or have insulin resistance, in which case discuss getting on metformin with your doctors.
Discovered most of my problem with urinary incontinence was due to a mistake in the prescribed amount of dexamethasone. I was given 4mg and it should have been .5mg, now after coming down from the 4mg to .5mg thing are much better.
I do think I should be taking closer to 1mg with the Zytiga but would like some input on this. Thank you.
