Hi,

Sorry for being away for some time. Actually, I wanted some time to elapse so that one has adequate subject matter.

Since my last post in July 2020.

# PSA Journey >>Jul -258, Aug 8.7, Sep 2.8 , Oct .88,Nov .14.

# BRACA 1 &2 Negative.

# Current Medication>>>>>Degarelix , Densumab (monthly ), Abiraterone and Prednisone (daily).

My team of doctors have suggested that I go though a PSMA pet scan , PSA and MRI in Dec (six Months since July) to determine the state of bone mets. However they have recommended that Instead of Monthly Dose of Degarelix (Firmagon), I may switch to Lupron 22.5 mg , as a once in three months dose.

May I seek your comments on this please.

Lupron worked well for me holding T at 7 until I opted for Orchiectomy and no more first line ADT drugs for life... Your numbers look great with everything in the right direction... Best of luck with keeping then that way.

Don't really know. Can't compare. Just no lupron for life and less worry about cardiovascular problems in ten to twenty years--dreaming of living that long. Also no scheduling of shots (nearest clinic 32 miles away in larger town). Do have all the no T symptoms--less hair, no stink, and less stamina.

Doclam2017, Absolutely fabulous Nadir PSA....0.14. Icing on the cake. .No BRCA mutation .Sir, You can continue to serve the motherland in some manner .. for at least 20 more years...(my sincere wish)

There are dozens of well done studies which indicate that a Nadir PSA of 0.2 or less with a time to Nadir above 9 months...gives a median survival of at least 6 to 7 years. With comprehensive treatments and modern technology this can possibly be doubled. Two studies in particular stand out are ..Morote et al and Maha Hussain et al about Nadir PSA ,TTN and prognosis.

Please do go for PSMA PET-CT as this scan is absolutely the best scan as we see real cancer cells and nothing but cancer cells...directly. I will be in Delhi in next few months and will get one scan myself. Vande Mataram.

Congratulations. Arc of your life journey reminded me of Ancient Hindu Scriptures telling that we, humans have four phases of our life.. first is up to 25 yrs of age when we learn all kinds of skills to do well in life...Second phase is from 25 to 50 years.. when we enjoy all pleasures and comforts existence has provided...third phase is from 50 to 75 years.. when we start to DETACH from loved ones, our possessions and our desires and longings.. And last phase starts at 75 yrs. when we become ready to merge with our creator and be one with the total. I am in phase 3 ... (age 62) But, I find it very very hard to detach from loved ones ,attachments and my possessions.

Yes I too am in the “forest dweller”, vanaprastha ashrama. And delight in doing so. Indeed I do live I’m a forest in Bend Oregon when I am not living at a beautiful beach in Los Cabos. And my working life has given way to periodic volunteer projects. And my highest service to others is teaching a deep authentic meditation practice which supersedes working for money in my profession. Some call it retirement but it is not renunciation nor withdrawal from loved ones nor community. I think Indian philosophy came to be dominated by the renunciates (sadhus, sannyasis) as the only trajectory of spiritual depth and fulfillment. And this is simply not true. The highest spiritual unfoldment is equally available for the 99+% of us who live the fully engaged “householder” life and contribute wisdom (as you do) and are profoundly committed to love to our loved ones throughout all of our days. Exactly so! It is called vairagya, an artfully crafted middle-path of gracious but present non-attachment. The word comes from vai or vi (outside or away from) + raga ( worldly desires and aversions). Excellent LearnAll.

3 month Lupron is still not available yet from what I can tell. I am monitoring a website that shows it backordered as I hope to go back to it end of Dec for my next injection. They substituted Eligard and I preferred Lupron.

I am on Firmagon for 26 months and continuing for another 10 months. I inject the Firmagon myself. It's a little scary first time but no sweat thereafter. Enjoy your journey to wherever it takes you - but you can always get off if you don't like where it's heading.
