Diagnosed in May of 19, worry about BCR , what to do?

I was diagnosed with pristate cancer in May of 2019 and had a prostatectomy with Robotic surgery on July 2, 2019. I had a biopsy before this and showed 5 of 12 cores cancerous at 2,10,20,20 25 % on 1 lobe with Extrapostatic Extension to the capsule wall. Perineal nerve ivasion as well. Gleason of 4+3 7 intermediate agressive. T2c After surgery path said T3a NO. No SV or LN invasion, PC got as far as the capsule wall. Not outside it.. I had < 5% tertiary gleason 5

Dr said he got it all, and I would not die of prostste cancer. Last 3 psa tests show <0.1..I worry about BCR...anit bothers me.. I haveto deal with the incontinence 2 pads a day and ED, wife is supportive . Just the worry about comming back.

Are you doing the Kegel exercise? [1]

In terms of preventing biochemical recurrence, my own view is that lifestyle changes may help. Lowering BMI might help. You don't look fat, but higher BMIs are associated with a greater risk of recurrence. A Medierranean-style diet (40% fat) might help. Many men with PCa should worry more about their heart than PCa mortality.

Enjoy life. Fill your day with activities that block out morbid thoughts.

There are a whole lot more things to worry about than BPR. Covid 19, a car wreck or heart attack all seem more likely and immediate. And even if your PSA does start rising, don’t rush to get into treatment. Read this: ncbi.nlm.nih.gov/pmc/articl...

So far, so good. In the future, ask for "ultrasensitive PSA" tests. It adds another decimal point - lowest is 0.01. Taken every 3 months, that will give you your best indicator that recurrence is starting.

Patience....Kegals, exercise, diet. ED should improve with time and there are many exercises you can do. Quarterly uPSA, and keep an eye on the pattern.

Your doc is correct, as long as you control that needless worry. Good luck.

I weigh about 195, most weight is around my middle , not a big belly though, lol. My Doc said we will cross that bridge IF it happens. I get another PSA Test first week of July. I will keep you all posted. Thanks for the input.

Hey sgtmac52d!

Your doctor feels that the operation successfully removed the cancer. If you have confidence in him try to be a little more at ease.

Diet, foods TO eat and foods NOT to eat! "Forks Over Knives" by Dr. Ruth Heidrich or there is quite a bit of YouTube video on her. I have been a vegan for two and a half years now. It was difficult. I started by reducing the amount of animal foods and products, then eliminating them one by one. Some foods are the cancer killers.

Anti-inflammatory natural supplements are good.

Look for Dr. Laurence Klotz, Dr. Charles "Snuffy" Meyers and Dr. Greger's YouTube videos.

As others have mentioned do Kegels. If all fails there is the AUS 800, the automatic urinary sphincter. As you are only using two pads a day the Kegels may work for you.

I would also emphasize the need to exercise. It will serve you well whether you have a BCR or not. I had a BCR after prostatectomy and, because I was walking 30 miles a week (age 72), I had virtually no side effects during the radiation treatments that followed.

I am 65 I live in Sharpsburg Md. , I was Diagnosed with PC in May 2019 , had a ARRP in July 2019, My Biopsy was 5 of 12 cores Positive, @ 2,10,20,20,25 % 1 lobe. T2c with Extra Prostatic Extension (In Capsule) with Perineal Invasion of Nerve. My PSA was 4.5 pre-surgery. Biopsy showed 1 lobe but Dr. gave it a T2c Eval. after Biopsy. My Dr. Name is Kambiz Tajkarimi ( Leesburg Urology Consultants in Va.). After my Surgery, Path Report was Clear Margins, No SV or LN invasion of cancer cells. I had a Tertiary Gleason 5 <5% (Bad Cancer Dr Said). Path report gave me a T3a NO rating after surgery. I have had 3 PSA tests after Surgery, Sept19, Dec19, Mar20. all were <0.1 with one in Dec with the VA @ 0.012 precisely. I get my next one 1st week in July be a year. My Dr. is very good, has a 99% cure rate anyone he does surgery on. He told me your cured of Prostate Cancer and your not gonna Die from it, His Exact words. But the Tertiary Gleason 5 is what has me worried.

Just trying to deal with it all. Looks good but Dr. testing me every 3 months

Playing the 'what if game' ? You might as well already be there, so what you would accomplish is all the worry - for what purpose ?

The mental part of illness is a tough one to manage - but assuming you are already 'cooked'basically makes you powerless.

Why not assume that you'll heal - like many others before you - at least it is as realistic as expecting to fail ..... and you are still a candidate for optimism.

Cooked ..ok? Lol..well I have good and bad days thinking the what if game...but I try to really not go there. I am not a quite and never will be...I am believing what the Dr said and trust that. As of now Iam cancer free...so will cross the bridge when I get there with a BCR..if it even happens . Betting it will not!! Thank you
