Briefly giving a summary dad diagnosed in jun 2014 as CA prostate Gleason 9 followed by RRP. Histopath showed clear margins with one oeriprostatic node positive. No adjuvant therapy but observation. Psa increased to 2 from .08 in 2 years with a solitary met in femur. Irradiated with ADT started in feb 2017. Psa remained <.008 when it has started to rise since nov 19. Now psa at .092. Pet psma gallium done showing a tiny lesion with minimal uptake in right humerus. Scared for dad. Please suggest the next steps. We are in India but in touch with dr eleni for same.

That is a very small PSA increase. It could be due to an infection or inflammation of prostate. If it continues to rise the same area can not be irradiated. However, xofigo is an option.
With PSA 0. 09, with a tiny clump of possible cancer cells...you need to relax. In every human body there are tiny group of cancer cells and mostly the immune cells kill or eat them up.
If you start chasing every little,tiny clump of suspected cancer cells, you will not have time to do anything else in life.
But, keep monitoring the PSA every month and if it start touching 1.0, may be that is the time to take action.
Have you considered adding abiraterone?
Scared about the prognosis. Solitary mets in humerus post ADt. Castrate resistance?
