Opdivo-

Was recommended in a second opinion appointment today.

Does anyone have experience with this drug for very advanced prostate cancer when other drugs have failed?

Thanking in advance for any experience with this drug.

You and I are in the same boat. Going to University of Chicago Hospital Friday to look into clinical trials and see what they have to say. Ironically I just started Medicare this week.

As you probably know, it is an immune checkpoint inhibitor. As a class, they have limited value with PC. A few respond well, but not many. Those with MSI-hi/dMMR, which is very rare, respond very well.

My husband was on it. He is dictating this. He was only allowed to get 4 treatments. The last one brought on a rash and he itched. I hope that it works for you.

I don't have any direct experience, but you might glean some information from the Full Prescribing Information for Opdivo, available here:

It has a longer history in several other cancers besides prostate cancer, and there have been more advanced Clinical Trials, involving larger numbers of people, with results of how actual human bodies react to Opdivo, either by itself, or in combination with other things. Look for things like Dosages, Adjustments to Dosages, Warnings & Precautions, Adverse Events, etc.

I think I'd also want to be asking my prescribing doctor about the Microsatellite instability-high (MSI-H) or mismatch repair deficient (dMMR) status of my recently biopsied tumor tissue, and also any results indicating the extent of expression of PD-L1 in those tissues, if available. Although clinical trials continue, perhaps Opdivo might possibly work better for those indications in certain advanced prostate cancers, too?

Has your MD indicated you have neuroendocrine or small cell carcinoma? I would think that might be the reason he is talking about Opdivo. Are you taking chemo? Is it Etopsel? Are you receiving Carboplatin? This is what I’m faced with but Opdivo has not been mentioned.

Have done Taxotere

Carboplatin and cabazitaxel

Zytiga Xtandi also- and Xofigo

All of the above.

Been battling at stage 4 for 4.5 years.

There is Provenge for soft tissue tumors. Also Casodex is another ADT drug.

We talked to a Dr. who works with Dr. Tagawa at Weill Cornell and she said Provenge helps when given early after diagnosis, and I think she said not after becoming CR.

I don't want to disagree your doctor but I was given Provenge 22 years after diagnosis. That was after failing Casodex. However I had not had zytiga or xtandi.

She's not our doctor, she was someone we spoke with concerning a clinical trial. If you're not CR (castrate resistant) yet, then that's in line with what she said. But if you are CR and received Provenge after you were castrate resistant, and it's helped you, then it appears she was wrong. Regardless, after all the research I've done, I've learned to not let statistics stop me. Every person is individual. Thank you for sharing this, and a huge congratulations to you for continuing to do well after 20 years. That's wonderful!

Yes-

All of the above have been done.

Not neuroendocrine.

Correction - have not had Etopsel.

They use that with Carboplatin if your cancer has mutated to small cell carcinoma. That is what I have right now.
