J Stent: How long can men keep using stent if TURP is contraindicated? Stage 4 man with mets.

I am a new poster but an old lurker. I was dx Stage 4 16 years ago with Bone Mets and told I had 6 months to 2 years to live. I just retired from being a social work consultant in a school district and up until now PCA has been a virtual disease and my quality of life has been good.

I am 74 yo Stage D2 Androgen Independent with PSA of 60 and climbing on Lupron. I have a large prostate that is pushing the ureter out of shape and caused hydronephrosis and typical BPH symptoms. Estrogen treatments were successful up until 3 months ago. I have J stent in the right kidney and now must decide 1. Remove the stent and see if the urine backs up again. 2. have the Stent replaced every three months ad infinitum, or 3. Have a TURP procedure.

I do not know what to do. Stents and hospital admissions can lead to infections.

TURP on a stage 4 prostate might be problematic?

WHAT SAY YOU?

choice 1 remove stent and pray

2. Get a new stent every three months?

3. Grind out the prostate and make it smaller so it does not screw up the bladder?

My stents have been in for a year and a half and are changed every 6 months roughly. I was advised that removing them and potentially putting them Back in can cause kidney problems.

Stage 4 no surgery.

I was stage 4 and still had surgery. Eliminated pain while moving bowels.

Wow tell me more. I am 74 dx 16 years ago stage 4 from the start Gleason 7-8

In trouble now PSA rising hormone insensitive will be doing zytiga. I wanted them to remove prostate 16 years ago

There are some new BPH treatments that are not as invasive as surgical TURP. I don't know if available where you live. But look into HoLEP, green light laser, REZUM, arterial embolization, and Urolift.

Hi TA. I am 10 months post HIFU. Quite restricted urine flow and 150ml retention. Uro suggesting standard scrape out TURP. Asked about REZUM (Imperial London) but he said they only do the standard TURP after HIFU. Have declined treatment until after November mpMRI. I will ask about possibly injecting collagenase via cystoscopy - has this been used before? Can't shove a catheter in - either too restricted or a cavity in there. Scraping and adding more scarring seems wrong. Any suggestions?

I spoke to a patient once who had a Xiaflex injection - you may have problems finding a urologist willing to do it and I doubt that NHS will cover it.

I'm always disappointed by what they can do and what they will do. But I'll ask. If he's up there anyway might as well stick in a few jabs.... I'll ask if the cystoscope comes in children's sizes. Eyes already watering...

Hi TA...what are the general guidelines for prostate embol-ablation? I’d like to punt my defective prostate..then through a farewell party. In truth I’m not having much trouble with the plumbing as the ADT seems to have shrunk it to a manageable size..but I don’t trust it’s threatening dormancy.

I had urinary stents in and out every 3 months for several years.... Make sure doctor prescribes antibiotic after insertion.

They can be very painful sometimes either when urinating or sometimes just a back ache around the kidneys. Also the hooks that attach it can tickle the bladder causing more runs to the bathroom.

Yes. After a two mile walk my urine is brown. Plus pain sometimes.

Hi Charlie, I was diagnosed in Dec 19 with Stage 4, GS9, PSA 50, 3 pelvic mets and had to have a retrograde kidney stent put in as my PCa invaded into my bladder which grew over and blocked my left ureter.

I was put straight on Casodex and Eligard and currently finished my 4th of 6 Docetaxel chemo sessions. This combination of treatments has managed to shrink the size of the tumour in my prostrate by 32% and reduced my PSA to 2.7. I am hopeful that this trend will continue and I will be able to get the stent out in 3 months as the chemo beats back the tumors.

Perhaps Docetaxel could be an option for you to consider with a view to reduce the size of your prostrate. Keep up the fight brother, 😎 regards DD.
