Why is the UCSF oncologist against it? Are your bone mets lytic?

What is 'lytic'?

Some bone mets eat away at bone tissue - "lytic." With most kinds of PC, the opposite happens - there is bone overgrowth causing them to become brittle and break - that's called "blastic."

I'm no doctor, but I might guess that you are getting the Olaparib because your genetics is a good match for trying this drug first. With the right genetic mutation, the response rate is relatively high, as cancers go, and they might first want to try to get the most out of it as they can, without confusing the situation with another concurrent treatment. See, e.g., this recent European news report.

As for the Xofigo (Radium 223), there are definite potential side effects, and various precautions and warnings, depending on individual circumstances. The overall response rate and any extra statistical Overall Survival time vs. the potential side effects might lead some doctors to hold off on it. See, e.g., some other European ideas on Radium - 223 suitability in some cases.
