What about requesting it after 3.5 years on Lupron, 1 on Casodex and 2 months on Xtandi (rough time estimates) w/a nadir of .92/peak of 4.2 PSA? Am I overboard?

Why would you want a more toxic combination that has no demonstrated efficacy for men in your situation? Most insurance will want evidence that you've failed docetaxel first.

failure=Progression in spite of treatment

They usually want to see radiographic progression as well.

Fellers, I'm in new territory here, in that I just recently began to focus on m own advocacy at the expert nudging of my lovely bride. I am quite frankly nearly ignorant about PCa; I've just been to busy of late to jack with it to the extent I need to be. Hence all the first grade questions.

Most of us here start with close to no knowledge. I would hate to think someone would not ask a question because they’re embarrassed about their lack of knowledge. My experience has been we are learning as we go. There is such a tremendous amount of knowledge from the members on this site. I hope you will ask away. People want to help.
