So my dad has been on Zytiga 1 month since a spike in PSA and a bone scan that showed it has moved basically everywhere
1. PSA went from 13 to 2.8
2. Alkaline phosphate readings have gone from 155 to 285 to 455 to 608
3 weight has gone from 224 to 211 ( not sure how to digest this he is eating cleaner so not sure if it’s life style changes with diet or the bone cancer )
Any similar stories or advice

I see NPfisherman response below. I'd listen to your Cleveland Clinic Docs, not me. Hope the PSA keeps going down & the weight loss is good. I lost 14 pounds when I started Lupron(exercise + better eating). A year+ later and the lost pounds are searching & finding their old home.
Wow, I go to Cleveland Clinic also... maybe they have seen the bounce described in the article I posted you...from reading it, it sounds like it is not a bad thing at all...
Hi .. my Dad started Zytiga on Jan.31. His lab results from 2/22 show that his Alkaline Phos tripled since being on Zytiga. I read the article that Fish sent - very interesting. This is so frightening wondering if he is receiving the right treatment. He’s scheduled for more labs on March 8th. Edbl, did your Dad’s go down ?
I just stopped Zytiga. PSA went from 20 to .2, but ALT was at 2.5 ULN and climbing. Now weaning off progesterone. Oncologists agreed that since I was only 6 weeks into initial treatment, transitioning to docetaxel made sense rather than trying to fight with Zytiga so it's off to Chemo next week! (whoopee... NOT) Funny, but I lost weight on Zytiga as well.
Was getting 25% drop in PSA per treatment. After 4 cycles test show rise before #5. Back to 0.205. Added Xtandi and went back to consistent drops for next 6 months. Including 4 months after end of chemo and just xtandi.. Had to cut Xtandi acct excess toxicity. Settled at 86 mg average daily dose. PSA came back up a little. Settled at 0.120-0.130. Just my reason for change at 0.2 PSA.
