Could I be failing already ?

My story with PCa started in April 2018 when I felt my peeing pattern changed.

weak stream, feeling I didnt void etc.

on June 2018 I started Lupron and 3 weeks after that I started Zytiga.

About a month ago I did some lab tests and PSA was 0.12. At the same time I felt a noticeable improvement in my peeing.

There are already a few days that my peeing pattern reminds me of the pre-diagnosis issues I had.

Could my situation have changed so quickly ? from feeling pretty good about a month ago to having symptoms I had before I even started the treatment?

What did your biopsy show and what stage? Zytiga works for some and not for others but there are other options, depending on many factors. You didn't give much information.

you're right...

GS 9 locally advanced PCa. its confined to my pelvic area. int/ext iliac ,presacral ,mesorectal lymph nodes.Dont have a formal staging but my understanding is that it is T3cN1M0

PSA at dx 21.lately discovered I have a BRCA2 mutation.

As of a less than a month ago Zytiga seemed to be working very good and actually my question was if its reasonable that judging only from my urinary issues the situation could change so rapidly or should I attribute it to something else like UTI?

I don't want to sound negative but my understanding is that the BRACA2 increases the likelihood of more aggressive cancer. My husbands was very aggressive and Zytiga worked for only 3 month before things changed so I would agree with you, change in symptoms does usually mean something. I would ask your doctor. Did you ever get radiation? With aggressive cancers like yours they usually do something like surgery or radiation/chemo as early as possible. Best of luck.

The initial relief may be from the prostate shrinking in response to hormone therapy. What was your original prostate size? Did you have an MRI that found enlargement in the transition zone? Your current obstructive symptoms may be due to a UTI, or other reasons (e.g., clots, stones, etc.). If it continues, your urologist may want to perform a cystoscopy to get a closer look.

The prostate volume according TRUS at DX was 23cc and dimensions 48mm * 33mm * 28mm but an unco Urologist who did DRE estimated it as 45cc

I havent done any imaging tests besides the TRUS and PET/CT PSMA
