had radiation 10 years ago.... zoladex for last 6 years with casodex, psa now going back up, should I be concerned? PaulN

Paul, Sorry not able to help - but hope the experts will be along to do so soon. Really looking forward to reading the replies you get as my hubby is in a similar position but in his case he had radiotherapy 5 years ago followed by 2 years of Zoladex. 18 months after stopping Zoladex his PSA started rising and a scan showed it had spread outside the prostate. Zoladex was re- started with Bicalutamide. Now 18 months later his PSA has again showed small increases at 2 routine consecutive appointments. Awaiting results of bone scan and, like you, we are concerned. Lots of good luck wishes to you.

PS. Has anyone found that either Zoladex or Bicalutamide has caused atrial fibrillation?

It's called castration resistance. Time for second-line hormonals. What's your PSA? Any evidence of metastases?

First thing I would do: stop Casodex (Bicalutamide) if you are still on it. You might get what is called a "withdrawal response" where the PSA goes back down again. After that, if PSA continues going up you need to add a "second line" ADT drug such as Xtandi or Zytiga. Then see what happens from there. I would do imaging if you haven't had any recently to see if there is progression.

Hi Paul.

You could try to take a "holiday" from casodex and zoladex. After that there is usually a better response to the treatment again. Someone said this in an earlier post. However, another and perhaps safer option is to go to second line hormone treatment straight away since you have been on casodex and zoladexfor a long time and maybe a holiday from it wouldn't even make it work better again. I think if I were you I would go to second line treatment.

Same thing happened to me. I dropped Casodex and switched to Nilutamide 150 mg daily. PSA back to undetectable in 3 months. Good Luck.

Casodex stopped working for me after 9 months. Onoc. switched me to Nilutamide and have been -0.1 for 3 yrs. Still on the Lupron.
