Dental Issues with long term survival?

I am having multiple cracked teeth and dental infections after 20 years of ADT and continuing chemo. Anyone else?

I just had my first fractured tooth after 3 years of ADT. This situation is a problem when one is being treated with zoledronic acid, prolia or xgeva. since extractions and implants when taking these drug are associated with osteonecrosis of the jaw.

Yes, after 20 years of treatments, see my profile for a summary. Dentist refused to do a regular cleaning wanted to do a deep cleaning which I declined. I am 75 and never needed one before, plus I am worried about it increasing the risk of necrosis. I had an abcess which he prescribed an antibiotic for and it cleared up. Dentist said I needed a root canal and crown on the tooth with the abcess which I haven’t scheduled yet for fear of necrosis. Not sure how to proceed.

Same problem. My husband had a tooth that lierery just came a part. Mo says no dental work while on chemo. So there is the hole where tooth was. Another on is cracked.

I would like to know the MO's reasoning--as well as what he/she is prepared to do if your husband develops an abscess.

Maybe there is a reason, but I can't imagine what it is. I've never been told this about any of my patients on chemo.

While it's possible to develop MRONJ after deep cleaning, it's not common--and in fact MRONJ can occur spontaneously. OTOH, I find often that dentists recommend "deep cleaning" in the absence of significant periodontal disease. Often this prescription has more to do with money than anything else, sad to say.

OTOH, delaying a root canal treatment for fear of MRONJ is IMO a mistake. You're almost certainly at greater risk for infection than for MRONJ.

This is a known issue that's usually never discussed. 80% increased risk of periodontal disease among men on ADT.

Shen: You have posted here twice -- once after 17 years on ADT and now after 20 years. You have never replied to any of the many replies you have received requesting your medical history. Having been on ADT for more than 5 years, I for one would love to read it. Thanks.

20 years ago at least I had gum disease and decided to do some home remedies. I found a tooth whitener that used hydrogen peroxide as the active agent in a gel form that came with a mouth guard. When I started using it I had some stinging in my gums because they were in such bad shape. But I persisted using it three days a week. Eventually I cut back to one day a week and used it through the length of a shower. It knocked out my gum disease and also whitened my teeth. My dentist’s dental hygienist acted astounded by the Change in my gums. The overall health of my teeth improved drastically. The brand is plus white gel and can be purchased online at Walmart. Be careful at first because your gums and teeth may have a similar response, that is to say you may experience some stinging. Keep your sessions short at first.

I know it's been a while but I would like to thank you for your suggestion. I tried some plus white peroxide gel and it worked exactly as you stated. My gums and teeth were in pretty decent shape before ADT but this really helped to tighten them up and avoid tooth decay and periodontal disease. I still use it about once a week or so for around twenty minutes.

I've seen no convincing evidence that ADT causes decay. I've seen evidence that ADT can worsen periodontal disease, but I've never seen a significant worsening in anyone who did not have pre-existing periodontal disease. This is just anecdotal, but I've been watching this in my patients for a long time.

20 yrs of ADT ? God bless your soul... amazing . I’m going on year five . Any suggestions for me ? I too am concerned about dental issues . I just had my first prolia shot while I’m in osteopenia .. my belief is that adt eats muscle bone and teeth ..the whole enchilada is effected ..Great job persevering .. Scott

My dentist took great interest in my cancer Dx and took steps to educate and prep me for long term treatment in mind.

Bacteria in the mouth CAN cause some severe complications if your oral hygiene is negligent.

Be careful. The collateral damage could be significant.

I am fortunate, seeing as I has ALL my teeth replaced prior to treatment with crowns.

Sure, it was an extensive and expensive way to move forward, but I am set for life. My teeth look like they belong on a movie star. That was the added bonus of an improved outward appearance.

Shen Stone. 20 years on this older post. 22 years now! I guess you know that is so rare. I refer to you guys as Jedi Masters! We can learn from you, we just need to listen well.

What has been your journey?

What do you feel was your best protocol?

Did you hv Surgery or Radiation?

You mention Lupron, what else?

I am 67 and only 8 mos since Dx. Stage 4 w Multiple enlarged lymph nodes, and one possible bone met. 7 turndowns by 7 Surgeons for Prostetechtomy. Gosh, I would take 20 years in an instant w no further thought.
