Hi,

I'm a 60 yo male with Advanced PC. Diagnosed in 2012. Underwent Proton Therapy at MD Anderson in 2013. Tested in September 2015 showed PSa rose to 38 and tests found metasteses into one lymph node in my pelvic region. Began monthly shot 240 ml of Degarelix November 2015 reducing my PSa to 0.5 in May 2016. Started a clinical trial in February of 2016 supplementing with Cabozantinib. Started at 60 mg, side effects became unbearable about 8 weeks in. reduced to 40 mg and have been able to tolerate reasonably well. Recently my PsA started to rise again, currently 2.3. Undergoing bloodwork Nov 2 to determine whether they will change to a different medication.

If Degarslix was reducing your PSA, why did you introduce Cabozantinib, now? Also, your pelvic tumor how much in measurement or percent was it reduced, by Degarslix. Do you go every 3 0r 6 months for scans?

The Cabozantinib was a clinical trial. It started when I started on degarelix to test if it would prevent the cancer from moving to the bones. The lymph node decreased by 50%. The trial has me doing scans every three months.

Did you have radiation to the lymph nodes originally, and that is why you could not use radiation again to treat the lymph node?

No I didn't I just did proton therapy.

I didn't treat the lymph nodes either, when I had 3D radiationn to the prostate. Now, I am thinking of having radiation to the lymph nodes.

According to my scans only one pelvic lymph node was affected. It has since shrunk more than 50%. They haven't seen anything anywhere else. My PSA started to rise in May. In November it doubled from 2.8 to 4.3. Took me off Degarelix and put on Lupron. Also the Cabo has inflamed my thyroid and it stopped working. So I have to deal with that until it comes back or they decide to put me on Synthetic hormones.

Why did you not think of spot radiation to your pelvic lymph node? I really would value your opinion.

Actually I didn't really analyze it. I felt the Cabozantinib was a step up in standard care. As long as the lymph node did not increase in size I would wait on it. Can't say that is right or not.

Thank you for the feedback. I am trying to decide to use spot radiation or robotic surgery on the lymph nodes {2}; or leave them to be treated by ADT and Lycopene. I too got PCa when I was 56 years of age.

It's a tough decision. I wouldn't offer an opinion because we each have to choose what feels like best direction for us. Talk candidly to each specialty and get the pluses/minuses. Then pick a direction AND don't look back. Blessings, Steve
