So I have just read about pulsatile tinnitus and it would appear that this is the answer to my left ear tinnitus when I am trying to get to sleep. Any point me mentioning it to the doc? Seems quite an innocuous thing to take time to worry about.

Are you sure it's pulsatile tinnitus and not middle eat myoclonus which is what I have at night? The former is like a heartbeat, the latter much faster?
Do you sleep on your left side? The main artery to the brain is very near your ear canal and this could be what you are picking up as head noise. Can be engulfing in the silence so try using a Sound Oasis box from the BTA.

If it is pulsatile T, there can be many underlying causes so I would see doctor and get referral. I am currently going through this at the moment.
