Hi In my experience a lot of my medical notes are wrong, either vital things omitted or .,much worse, extra dangerous things put in. I have a lot complicated and dangerous conditions so these is especially important for me. I keep a "proper" list of conditions, drugs, history etc on the PC, When I see a new consultant and always if out of the house or in an ambulance in an emergency or hospital admittance. I take a print out. When I see some one new, or, need to remind them.I say " I have a copy but with a GP`s print out you may not want it!" They have always said that they prefer my accurate notes not GP`s ones.Even, the best hospital that I have to go in ask in A and E for it.I actually then, take recent letters ,if very important.

I think you are very wise and we should all take note.
I worked for a doctor for 20 years in the Channel Islands. Each day the notes (no computers then) were taken out for each patient. Any allergies were listed in a prominent place and the patient was always asked if there were any others to be added since last consultation. The doctor then read the notes of each patient before they started the consultation.
When I came back to the UK after 40 years of this it was frightening. Language problem (my Chinese, Italian , Indian etc etc you name it they speak it) was not up to scratch, no notes, it was all up to me. I was a stranger each time I visited the surgery and if I didn't tell them everything then they didn't know anything about me. As far as I could see the only time they look at their computer screen is to check up on a blood test. Where all my beautiful notes I took to the UK surgery with me are now I have no idea. I assumed they would have been put on computer but when I mentioned I had Hashi's I was told "no record of that" so I assume they know diddly squit about me and the notes got filed conveniently in a skip somewhere. I changed surgeries and the new doctor (who I do trust) has no idea of my previous medical life and we are working from scratch. Fortunately...being that sort of person, I had taken copies of stats, specialists letters etc so I had saved a bit.
I now keep very strict records myself .
I am a pretty tough cookie and can well look after myself and will fight my corner but when I think of my mother and my father and what terrible times they went through because of bad medical care I could weep. I did try, with my sister, to do something about it but it was useless. The medical profession look after their own even though they lack the same consideration for their patients.
Hi Thee difference is amazing. I also find that with even the brightest, best doctors any thing remotely relevant needs to be mentioned each time!
