Hi

I’ve just received my test results from Blue Horizon,I paid for the advanced profile.

A quick background….I’m 54,have been on 75mcg levothyroxine for years,but for a while ,haven’t felt like I was cooking on full gas….fatigue etc.

I appreciate this could be age related !!

Got no help from GP so decided to be proactive and get my bloods checked.

A few things were high,not really concerned about my Vit D/B12 being high as just shows my supplements work! Cortisol a bit on the high side ….

The main thing I would like some advice on is the high anti-thyroglobulin antibodies. What does this mean please??

Any help and advice would be greatly appreciated.

Presumably you did test early morning, ideally just before 9am, only drink water between waking and test and last dose levothyroxine 24 hours before test

This gives highest TSH, lowest FT4 and most consistent results. (Patient to patient tip)

Free T4 (fT4) 14.7 pmol/L (12 - 22) 

Ft4 only 27.0% through range

Free T3 (fT3) 3.2 pmol/L (3.1 - 6.8)

Ft3 worse at only  2.7% though range

TSH too high

Many people when adequately treated on levothyroxine will have TSH around or below 1

Aiming for Ft4 and Ft3 at least 50% through range

Request “trial “ increase in Levo to 100mcg daily

Initially suggest you ONLY increase to 87.5mcg dadaily

Or alternate 75mcg and 100mcg for first 6-8 weeks

Then retest

Which brand of levothyroxine are you currently taking

Do you always get same brand at each prescription

Cortisol is high because your thyroid levels are low. This should improve as you get Ft4 and Ft3 higher

What were ferritin, vitamin D and folate results

About 90% of primary hypothyroidism is autoimmune thyroid disease, usually diagnosed by high TPO and/or high TG thyroid antibodies

Autoimmune thyroid disease with goitre is Hashimoto’s

Autoimmune thyroid disease without goitre is Ord’s thyroiditis.

Both are autoimmune and generally called Hashimoto’s.

Significant minority of Hashimoto’s patients only have high TG antibodies (thyroglobulin)

NHS only tests TG antibodies if TPO are high

NHS doesn’t currently recognise having high TG antibodies as definitely confirming autoimmune thyroid disease

TG antibodies often reduce as dose levo increases and TSH reduces

what were your actual vitamin D, folate, B12 and ferritin results

Ideally choose an endocrinologist to see privately initially and who also does NHS consultations so that might eventually transfer to NHS

Hi again

Sorry for the delay in replying🤦🏽‍♀️

So here are some more blood results:

Vit D =>300 (have been taking a high dose of Vit D/K2 for a few months)

Vit B12 =639(been taking a supplement)

Ferritin =77

Serum Folate =13.5

Magnesium =0.8

Iron=17.8

Are you in U.K.

units measured in nmol

Your result is far too high

Stop supplementing and retest in 2 months

Aim to maintain between 125-150nmol

Yes,I’m in the UK and Vit D measured in nmol.

I have stopped them already👍🏼

Is my Vit b12/ferritin/folate/magnesium/iron all adequate??

What’s the range on folate

Could be 3-20 or 7-60 or something else

Serum B12 over 500 - so that’s good

Ferritin at least over 80

How much vitamin D were you taking that increased levels so drastically

The folate range is 8.83-60.8

I was taking Vit D/K2 at 10,000iu,one per day

That was massive dose

We all vary significantly in how much vitamin D we need…..that’s why it’s best to start cautiously and retest twice a year

Some people only need 1000iu or less per day

Folate - Serum 13.5 ug/L (8.83 - 60.8)

Folate only 9.0% through range

So definitely needs improvement

supplementing a good quality daily vitamin B complex, one with folate in (not folic acid)

This can help keep all B vitamins in balance and will help improve B12 levels too

B vitamins best taken after breakfast

Igennus B complex popular option. Nice small tablets. Most people only find they need one per day. But a few people find it’s not high enough dose and may need 2 per day and/or may need separate methyl folate couple times a week

Thorne Basic B recommended vitamin B complex that contains folate, but they are large capsules. (You can tip powder out if can’t swallow capsule) Thorne can be difficult to find at reasonable price, should be around £20-£25. iherb.com often have in stock. Or try ebay

IMPORTANT......If you are taking vitamin B complex, or any supplements containing biotin, remember to stop these 3-5 days before ALL BLOOD TESTS , as biotin can falsely affect test results

In week before blood test, when you stop vitamin B complex, you might want to consider taking a separate folate supplement (eg Jarrow methyl folate 400mcg)
