TSH, T3 & T4 results Help!!!!

Hi I recently went to the drs due to extreme fatigue and muscle aches they sent me for a blood test and today I received a text message telling me my thyroid levels were low and I would need retesting in 3 months but no other information and I don't know if I should be concerned 🙈 Serum TSH level < 0.05 miu/L [0.3 - 5.5];

Serum free T4 level 19.0 pmol/L [12.0 - 22.0]

FREE T3

Serum free triiodothyronine level 5.3 pmol/L [3.1 - 6.8]

apart from my TSH being really low the others seem normal 🤷🏻‍♀️

could this be what's causing the fatigue???

Yes, it could be. TSH being that low is suggesting you might be verging on being hypERthyroid. I say verging on, because FT4 and FT3 being near top of range is why your GP is saying wait to be retested in 3 months to see if those levels go over range.

Did they test for thyroid antibodies?

Thank you for replying and no I don't think so it was mainly Vit D, B12, Blood Count, Liver Function, Ferritin, Urea and Electrolytes and TSH T3 & 4. is testing for thyroid antibodies a routine test?

Welcome to forum.

I’m sorry to say your GP message is wrong. Your thyroid levels (FT4 & FT3) are not low - not even in lower half of range. they are in range.

But they are likely higher than your normal or have been high or are rising.

A TSH (thyroid stimulating hormone) isn’t a thyroid hormone. It’s a pituitary hormone & signals thyroid to increase or decrease production of thyroid hormones. The signal is saying stop - levels are high enough.

Hyper levels can be transient. And can definitely be the cause of fatigue.

Because your TSH is low but FT4 & FT3 are within levels are are classed as sub clinical & the doctor is waiting to see of this is temporary.

3 months is the recommended repeat time but I think it’s too long & if you feel worsening symptoms , Go back. Or carry out a private blood tests retesting thyroid levels which you can opt for a more complete test,eg with thyroid antibodies and key nutrients.

Ask dr to test thyroid antibodies & inflammation markers on next test.

thank u for ur reply what test is inflammation makers?? I definitely don't think see any results that seem connected to the thyroid but they did test other things as well x

Drs usually test C-reactive protein (CRP) sometimes Erythrocyte Sedimentation Rate (ESR). For inflammation markers.

There are lots of thyroid antibodies drs usually start with TPO antibodies. Labs sometimes with also test TG antibodies-

TPOab (Thyroid Peroxidase antibodies). (Autoimmune - both Hashimoto’s thyroiditis & graves)

TGab (Thyroglobulin antibodies) (Autoimmune - both Hashimoto’s thyroiditis & graves)

If you have continuous hyper levels you need TRab & or TSI antibodies tested. Often this is done by specialist referral.

TRab (TSH receptor antibodies) - measures stimulating neural & blocking antibodies) (positive with graves)

TSI (Thyroid-Stimulating Immunoglobulin) (positive graves)

Was folate tested? What were ferritin, B12 & vitamin D levels. Drs only look for deficiencies but in range is not same as optimal.

Dr advised I take over the counter Vit D tablets coz it's low but not deficient

GP will often only prescribe to bring vitamin D levels to 50nmol.

Some areas will prescribe to bring levels to 75nmol or even 80nmol

Was folate tested?

2-3 weeks after starting vitamin D, look at improving B12

With serum B12 result below 500, (Or active B12 below 70) recommended to be taking a separate B12 supplement

A week later add a separate vitamin B Complex 

Then once your serum B12 is over 500 (or Active B12 level has reached 70), you may be able to reduce then stop the B12 and just carry on with the B Complex.

If Vegetarian or vegan likely to need ongoing separate B12 few times a week

Low folate

supplementing a good quality daily vitamin B complex, one with folate in (not folic acid)
